All...I'm three weeks post-op (11/8) and submitted a form request to my surgeon's office for all the relevant surgical notes from my procedure at exactly the one week point (11/15). I was told that the medical records office would contact me to finalize the request, provide the documentation and answer any other questions I had. To date, I've heard nothing. I know we had the Thanksgiving holiday during that time but I did, at the very least, expect a call saying they received my request and an estimated time when I would receive the information. I will give them until the end of this week and then I intend to begin calling daily until I get what I asked for.
When you all requested your notes, how long did it take to receive the information you were looking for?

Most hospitals I have dealt with contract with another company to handle their records. I find it usually requires a form to be filled out with a check enclosed to get them and can easily take a monthto 2 months. I have paid anywhere from $25 to almost $400 for records depending on number of pages. Some records were complete with surgical notes, anesthesia notes, nurses notes, doctors notes and tests. Some were grossly inadequate like Vaderbilt's which was 1 1/2 pages that could easily be considered a generic as only 1 or 2 sentences were specific to me and covered no details to specific my surgery. The EASIEST way I have found to get the info is to ask a nurse when you are at the doctor's office. A co-operative nurse will print them out for you. It takes days before such things as my lab work are posted on line for me to review so I always ask whichever nurse is prepping me for chemo if they will print me a copy. That way I don't wonder for days and I can calculate where I am at and determine trend line directions while getting the infusion. Some nurses will tell you to contact records but most will co-operate.

Thanks, ali and ron. Interesting how diametrically opposed two supposed medical facilities can be in their handling of similar requests. I intend to begin calling Virginia Urology's medical records office today and follow up with a letter to their Director of Surgical Operations and/ or CEO if necessary.

I always go in person fill out the form takes like 5 minutes, ten tops. Now I did ask for the notes for my office visits from my 2 surgeons. The one gave them to me on the spot the other still waiting that was also just before Thanks giving.
These are the 2 guys who screwed me up. The notes that I got from the one were pre op. Read the fine print and it warned that the doc told me "I could lose my penis, testicles and scrotum from the penile implant surgery." I must have asleep during that part of the movie. Anyone else get that warning? I did order some notes from Tampa over the phone. Still waiting on those but I had an appt later (250 + mile drive) and while there I got the notes in what 3 minutes? So if you can do go in person.

No problems that I know of but since this is my first and only implant, I want to know everything that went on in the OR. I have still not received any definitive information from Coloplast and the only info my surgeon gave me was the length of the cylinders and number of RTEs. He didn't even give me the type of Titan used. I'm getting the run around from the conglomerate medical record repository that Virginia Urology uses to 'store' and distribute patient medical records. I'm very pissed!!

You should have received a paper with three stickers on it that would have come off the package the implant came in telling you make model serial number size of the cylinders type of pump and size of reservoir with the amount of fluid in it. I got that info when I walked out of the surgical center
