Brother,
Most insurance requires documentations from your urologist. An implant is only done when you qualify. I had radiation treatment for prostate cancer and my dick was dead. An implant was my only viable solution. Insurance companies usually insist that you have tried everything else unless there is a condition such as Prostate removal or Peyronie's. They don't she'll out the money (mine was 94,000) unless they have too. Good luck my brother.

The surgeon’s office want your business, the insurance doesn’t want to pay unless they have to.
Pick the surgeon you want, call their office, ask what’s the best insurance for the implant, get that insurance, have that office process the claim and get your surgery done.

Age 54, Single, Los Angeles. Stage 2 rectal cancer in 2013. Radiation, surgery, chemo. In remission for 5 years. Some ED before cancer, complete since. 2-3 units of Trimix 40/30/2 2-3 times a week since Feb/2017 with great success!

Here in the States, if you have insurance they pad the bill. Then chop it in half. My out of pocket was around 800. No worries, mate.

Welcome to the site.
First of all insurance is state controlled so it differs a bit from state to state so location may help a bit. Not all insurance companies sell in every state so you would need to find out which companies sell in your state.
I have BCBS of Kansas Gold plan insurance and they have no out of area coverage for my implant surgery. As this is considered an elective surgery (meaning I will not die if I do not have this surgery) BCBS can say who they will pay for this. So just because a surgeon I choose accepts BCBS dose not mean I can go to him/her and have BCBS pay. My plan does not require a referral. Your plan may be different. What it does require is authorization that I need this surgery because of physical injury of damage. My local health care does not do any testing. They only do treatment. So I had to do oral meds and fail and injections and fail with documentation of this before BCBS would pay the surgeon(s) they choose for this. And as I said that the cause was from physical injury or damage. My BCBS policy does not pay 100% of this.
I buy my insurance through BCBS as I am retired. It is compliant under Obama Care. Some work place provided insurance may be different. And I have seen posts on this site where Medicare will pay for out of area surgery.
I would recommend finding a very good local surgeon for this if you can. Not only for cost savings but for follow up care. Much easier that way if you are one of the few that have problems to get them resolved. I would look for a surgeon you trust to provide you the best possible results. Sounds like you are on the right track with your local uro and having testing. Do some research on him/her and others in your area and see if you can find a really good one you trust and make a plan.
I did not have any good surgeons in my area. Only ones who not give me infection rates and said I would loose one inch. I paid out of pocket for mine. It was 24k plus travel and lodging. If I would have stayed local the service providers would have billed much more. Insurance would not have paid all of it and I would have had to pay somewhere around 7k.

I have Federal Employees Blue Cross and it looked like Dr. Eid was NOT on the list but his office assured me he IS on the list. I discovered that Blue Cross has regions and if you are looking for a doctor that is in a different region it may not display his name even if you search his state. Before you conclude he is not covered, call the doctor and Blue Cross and give Blue Cross his name, specialty, and every address he practices from.

If any surgeon, even a super star, told me I might lose length, I would run like hell. My local surgeon told me I would not lose length and I didn't. I couldn't be happier. Why would a perfectly healthy dick , like mine except for severe venous leakage, lose any length. I don't understand. My dick was a nice 7" erected and after my surgery and my Stretching Routine. I am still 7".
