For me, pointed down in tight Jockeys when wearing dress pants. With pretty much everything else, I wear thin polyester boxers — as close as I can get to commando and still have a layer between my pants and my butt hole .

When I dress up I wear fairly snug boxer briefs, with my dick pointing down. My dress slacks are not tight like some younger guys wear, so there is just enough room that my penis is not blatantly visible. If I look in the mirror I do not see a bulge, but I am sure when I am walking towards someone they can probably detect my shaft to a degree. My shaft, when not pumped is 5”. For guys with bigger dicks it would obviously be somewhat more challenging.

I guess it depends on what you are calling by a bulge. All cocks and balls create bulges if they are average to large in size. It’s not just those of us with implants. So, if you are asking about what to expect following your implant surgery, my experience was having a partially inflated implant for several weeks, resulting in an obvious erection. I wore baggy clothes to disguise that. Some guys have major swelling and their scrotum ends up the size of a grapefruit. I did not have that. I believe it goes down after a week or two.

Then there is the bulge created by our new hardware. That will last as long as the implant. I don’t know what your flaccid and erect lengths are pre-implant, but keep in mind your flaccid may be considerably larger afterward than it is now. That means it is most likely your penis will show more in your clothes with an implant than it does now. You can either choose to embrace the new look or change how you dress to hide it.

Hugh, that is just semantics. I think it is understood here what is meant just as fully pumped still provides an implanted man with an erection, although it is not natural. Meriam-Webster dictionary defines flaccid as not stiff or firm so my use was not incorrect. When my implant is deflated it is neither stiff or firm so it still applies, at least in my case. Personally, I find using the same words for comparisons is helpful. You will see when I am not comparing I often use deflated.

And I assume there is no way to know exactly what your flaccid penis will look like after surgery and how big it will be.

It would be awesome if the doctor could say,

"this is what your anatomy will look like post surgery with the AMS, and this will be your anatomy after the Titan"

...to include the bulge as well..

I'm not really sure that would be possible. I have an AMS and my dick only points down very slightly when deflated - maybe 20 degrees.

To conceal it, it strongly depends on which implant brand you have. AMS is, after 3-4 months, very close to what you had before. The Titan is another story. You will need to go through a trial and error period, until you get the best combination for you.

In a perfect world, or maybe I should say dream world, it would be great to get that information ahead of time. Unfortunately, we aren’t at that point. I am pretty sure no doctor promises anything. Mine did the flaccid stretch test and said that was approximately what my implanted erect size would be. Until the surgeon gets in there, specific size remains unknown. I ended up with a 4cm longer implant than my doc thought I would get.

In my preliminary consultations with my doctor he said I could choose between AMS and Titan. I chose the AMS LGX. After the surgery he said he was grateful I chose that model because once he got in he was pretty sure my anatomy was not conducive to the Titan’s cylinders.

I am no medical expert but I assume the way each guy’s dick will hang depends a bit on size, how it was prior to the implant, and how much of the implant device is internal vs. external. There are just too many variables to know much beforehand.
