kmeister, My records say NOT DOCUMENTED under every category. That is equivalent to not keeping any office visit records to me.
No treatment plan? No documentation of the prescription they gave me? First a lack of records at Vanderbilt blamed on staff, now no medical records at Rush University. What has become so hard so document medical records? How do they get paid by insurance companies if they do not keep any records of what was discussed, treated, treatment plan, prescribed?

All my implant records are intact from every single visit. They are not accessible to regular office staff. I had to fill out special request forms and wait for processing. This keeps the information more secure I'm told. It's even that way with the VA. This prevents casual access staff from sticking their noses into your business. If you want actual surgical notes/documents you probably need to contact the custodian of records and do the paperwork. At least that has been my experience thus far. The typical perso. Behind the desk sees exactly what you have posted.

Yes, I had to fill out all kinds of forms to get the records plus pay by the page for them even though they were sent by email attachment. Same with Vanderbilt. I've never seen such incomplete records in my life.
