Consultation with Dr.Milam

Postby tmwstw » Mon Jan 24, 2011 10:13 pm

Thanks every one for the concerns about my problems, especially Dave, Scot, Matt and Rich!
So i had my consultation with Dr. Milam today at the VanderBilt. The hospital and the university is very big, but the urologic surgery department is smaller than i expected. Todd and Dr. Milam are very nice people and put me at ease as soon as i saw them. The nurses are pretty!! And they took my Florida BC/BS with no problem.
As for the specific info about the implant, i have to say sorry, i cant give any details about this subject because it was not an option for me today. So i did not have a chance to ask more questions about it.
In my consultation,
1. Peyronie is confirmed, again!! ( But the bend is not bad enough to stop sexual intercourse )
2. Venous leakage is confirmed ( But still respond to Cialis )
Todd and Dr. Milam said it's too early to consider an implant for me at this stage even though, in their opinion, implant is the only sure bet i have to fix this. I dont have any plaques in my penis. In my case, Peyronie is caused by the scar and lost elastic tissues in the tunica itslef. Todd said only a penile implant can push out the indentation and give me back the girth. He then measured my length and told me i'm lucky to have 5.5" left after a Plication. They adviced highly against the "Excision and Grafting" Procedure, They said it could worsen my ED and cause more scar tissues to form. Todd also reminded me, if they give me an implant now, i would need more than 4 revisions in my life time. He said the 1st and 2nd is no problem, but after that, the cosmetic result will not be pretty. I think it makes very much sense!! We have only that much meat for them to cut on!! Dr. Milam ended up prescribed me with Cialis (Good news Dave!! i have a new prescription now!) and an Osborn Pump for exercising. They want to wait until the pills stop working for me and buy me some more time for less revisions needed. They encouraged me to hold up on implant althought they showed sympathy for my condition.I thanked them and left.
Looking at the diagnosis report today: Peyronie and Organic Impotence, at age 30 ... i cried as i walked to my car and as i'm typing this. It's gonna be a tough decision to whether i should follow up with my 1st surgery option or not.
Thanks again every one for the support from the chat room.

Phuc, I thought about you today. I can not say that I am supprised by your visit. I guess you can say it was one of the good news bad news events. Good that the pills are working for you bad that there is not a surgical fix that will take care of the problem for good. Yes i guess it is a long road. Many of us look to be on that long treck.

Hang in there guy (pun intended)

Keep in mind, Phuc, that the medical technology relating to ED is advancing on a pretty steep curve. If you can function w/o an implant for maybre 5 years, who knows what might be available at that time? Maybe new drugs? Maybe new surgical techniques? Don't forget that every step is progress, even if the news is not what you wanted. At least now you know what your options and the consequences of each might be. That is info you didn't have before.

We will continue to support you on this journey. Chin up, friend.

Phuc,
I am glad you were able to post your visit results so quickly. After I read your post, I came away with the feeling that this visit was a POSITIVE one for you in a number of areas! First, it is good to know, from knowledgeable sources, another Plication is NOT in your best interest. Whew, I was hoping they would tell you that! There is too much information available that suggests surgery to resolve Peyronie's, while effective in eliminating the curve, definitely will cause the penis to lose some length, and exacerbate the ED as well. Second, your venous leakage is confirmed, but you are still able to achieve erections with the ED meds, that is GOOD! Third, they advised the Osbon pump, another good recommendation. I have the manual Esteem model, any pointers you need, I'll help in any way I can. Daily exercise will help maintain penile health AND YOU MAY even stem the tide of additional loss in length. The penis being stretched wil help the curve as well, hopefully slowing the progression or at the very least, slowing it down! Fourth, you felt comfortable and at ease with these 2 guys, very important for you. You can trust them, they want what is best for you and it sounds as though their advice was sound, and well accepted. Phuc, I can't begin to imagine how I would have felt at age 30 and experiencing the issues you are living with . I know this, you are informed, motivated and ready to tackle this problem and I commend you and wish all success. Remember, we are here to support and advise as well as just talk, stay involved and use the collective knowledge of all the fine men who are here for YOU. Personally, I know you are in a good place, you will continue your journey, you are going to be OK! All the best my friend.
