One part of the surgeon's job is to measure the inside of the patient's tunica albuginea and select the proper size implant. This takes skill and judgement (usually gained only through training and experience).

Certainly the patient has a job to do in preserving length, and the patient NOT doing his job can undo the best surgeon's work, but no amount of proper patient activity can overcome a surgeon's mistake in length preservation.

Well, surely it takes a lot of skill. What else takes a lot of skill? Being a urologist who performs surgeries.

This is the way my doctor explained it:

Hi, I talked to my patient liaison from Boston Scientific and she explained to me (I think) that for my penile implant surgery you measure my penis size as it was necessary for the size of the implant. So, what is it? I imagine it's a sensitive question for a lot of men, but if its in the records l'd like to hear it. Thanks

His response:

During surgery your total penile length measured 24 cm which is about 9-1/2 inches. That is the total length including the part of the shaft goes up underneath your pubic bone in front.

I asked for some clarification because I obviously don't have a 24cm (9.44") dick, and he responded:

Your penis actually extends beyond what you can easily see even with it stretched out completely. There is a portion that goes underneath your pubic bone and in fact is attached to the pubic bone to help support the penis. We would put in the implant, we have to include that area in the total length of the implant. So while you are asleep we are able to place the measuring device down into that part in order to get the total length of the penis. So the length of the implant is longer than what you can actually see. In fact you may be able to even feel the implant in your crotch area where the cylinders go back along the penile corpora that far back. I know that is a confusing explanation but I hope it accounts for your measurements in comparison to what measurements we got in the operating room.

The way I interpret this is they basically measured my stretched flaccid, which was 8.75" pre-surgery. My normal flaccid size was about ~4.8" pre-surgery, but that isn't relevant to the implant procedure. My flaccid size now increased from ~4.8" to a bit above 7" bone pressed.

It's just my belief that the most common patient complaint has always been lost size, but that's not due to implant or the surgery.

Seems very possible. I guess there's only so much saline in the valve, and it probably wasn't prepared for almost 9". Something to worry about, but rigid definitely isn't the right word. I've been bending mine all directions, many times.

The fact that I have gotten measurements right around my pre-size of 8.75" makes me think I still have some room to improve. It's rare to measure it there and I don't see it often, but makes me think it's certainly possible.

The measurement the surgeon takes in the operating room is not the measurement anyone can make of your penis.

The measurement in the operating room is of the INSIDE of your penis, not the outside. The two measurements are related, sure, but they are NOT MEASURING THE SAME THING.

A loose analogy: A man can measure the length of his shoe from heel to tip and get a number. When he measures the INSIDE of the shoe, it will certainly be less (at the very least because of the thickness of the leather).

It is part of the surgeon's job to make sure there is enough saline in the reservoir to fully inflate the implant to get the appropriate inflated size of the implant/penis combination and the appropriate amount of rigidity. The standard of care in the operating room is to fully inflate and inspect the erection before finishing the connections. If there is insufficient saline to achieve the proper erection, more is added. Only then is the implant's tubing all finished, connected, sealed and the operation closed up.

It takes a lot of skill to play in the NBA. Does that mean every NBA player is as skilled as LeBron or Steph? Same logic applies to surgeons.

I'd certainly agree some surgeons would be better at this procedure. I've read studies mentioning how high-volume implant surgeons have better results. If I had the money/time I'd have gone to one of the best like many of you did. It's your dick, who wouldn't?

That's not a great analogy, though. If you make it to the NBA, you're pretty freaking good at basketball. I trust my local urologist I've seen from a few years just like I'd trust that the player on the bench for the Lakers as being incredibly skilled at the game. My urologist isn't as skilled LeBron/Stepth, but it took him a hell of a lot of skills to get to play the game.

I may have previously replied to this question, but I had no pre-existing issues that would cause shortening and right out-of-the-box, I was at my lifelong length. I did experience some discomfort in the glans that was suggested to come from aggressive sizing.

That turned out to be correct and I easily picked up a cm. If I hard press it, I can get 1/2" gain, but that's hard pressed that I don't remember if I used back in the day.

I was only a few months from RP to implant. I lost filling because dr removed all my nerves at RP.. Length is exactly the same. If I pump to Max I may add 1/4 inch but it hurts like hell. The tip curves up like the ipp is trying to break out. Definitely sized to the maximum length.

Honestly? I think I made a big mistake getting it. I was fine with injections and thought it would be great to have an erection at will and then deflate it instead of icing my balls till the Trimix faded, usually after 90 minutes and sometimes 60. I’m disappointed in how it feels fully inflated. Oval shaped and not able to engorge the glans at maximum inflation. If I go 85-90% and take a Levitra it almost feels like my pre-op and I’m working on getting some Trimix gel which is freaking expensive. Wife didn’t like it at first because the lost girth is what rang her bells. Now after cycling to 5.5” girth it worked for her but ached like hell for me. I can’t seem to come now unless I back off and masturbate to the trigger point and resume sex to finish. Can’t turn the clock back so I’m trying to get to a place where I’ll like it. I have to get a revision in the future to fix the doubled up and palpable tubing and wound scar therapy for the ventral chordee that formed on top of the transverse incision. I have a day or two of nagging pain from the stretching of the scar and the palatable tubing being manipulated during sex. So I’m trying to arrange to have Dr Eid or Dr Perito do it in January 23. Dr Eid was not very complimentary of my surgeon’s work. Sorry to be a bummer, hope it’s working great for you and I trust eventually i will too.
Thanks

Howling8,
I see your frustration with implant not being and feeling same as natural functioning penis even with aids like penile injection. You were functioning with TRIMIX which beside loosing some lack of spontaneity , still worked great.
That is why it is stressed here a lot to try and utilize every available option that works for having normal penetrating PIV sex. Once everything failed, IPP is the only option to continue sexual activities but it’s dead end street with no return back after feeling regrets.
For many of us here, Prostate cancer survivors, implant is God’s sent option to have sex life again. We did loose nothing by getting an implant, but we regain our sexual function back. I had prostate Ca at age of 51 and recovered nothing in 2 years period after RP when I exhausted all available options. Now, I have what I have and I feel like a man again despite my arrogant wife calling it dildo. Relationship was damaged in aftermath of RP. I would never ever married an RN again. Sad but thru.
