Just an internet dude here - I feel it's not too late, but it is time to make it a priority. I would also get aggressive with the Pierto (spelling?) exercise.
I also experienced gains after a year, but I was cycling with purpose and consistent at it.

To hell with money/out-of-pocket costs, insurance, and/or time - you want the BEST surgeon of the two. That's where you focus, let the rest play out.
That's what I think anyway. I am unfamiliar with the surgeons mentioned so I can't help you there. At least a solution is on your horizon either way.
