41 year old, 3rd peyronies lump - surgeon says implant

I’m 41 years old and 18 months post finding my 3rd peyronies lump, which has resulted in a total of 3 inches of length lost by all 3 lumps. The first two caused a bend to the right but my penis remained straight. The 3rd lump occurred on the top of my penis causing a 75 degree bend upwards. It’s also caused a hourglass effect. I can still achieve an erection but not as spontaneous as they used to be. Intercourse is difficult. Due to the size of the scarring, as well as the hour glass effect, my consultant in the uk has recommended an implant. What do people think? Do I persevere and not have the surgery as I can achieve an erection, or due to the severe degree of the bend and the fact I’ve had this 3 times make sense to opt for a implant?

Was the peyronies caused by trimix injections?.
I'd be going an implant for sure, the problem isn't going to go away by itself and if anything will probably get worse
43, ED since late 20's, Pills on and off since then increasing strengths with inconsistent results. Now 5mg Cialis daily and either 20mg Cialis or 100mg Viagra
Not sure if it's Psychological or Physiological, long-term use of SSRI's probably hasn't helped
