Good morning, brothers.

Although I’ve been actively participating in the Frank Talk forum through various threads for the past two months, I thought it would be useful to create a specific journal of my journey before, during, and after my surgery for the 22 cm Coloplast Titan One Touch inflatable prosthesis.

Before the surgery

I am 28 years old, just turned. I’m a tall, healthy, athletic, and hardworking guy, but ever since I started having sexual experiences (first with women and later with men), I realized there was an issue with my erections. Despite being young, my erections were often incomplete and I would lose them during sex. I had erectile dysfunction (ED) that only got worse as the years went by.

I started taking pills (Tadalafil, among others), and they seemed to work but only partially; they weren't a guarantee for excellent sex. I tried injections, but I didn't like having to prick my penis before sex—plus the pain and the bruising. My doctor also told me that long-term use over decades could lead to side effects on the penis, and they weren't a guarantee for a perfect erection either.

After visiting several doctors, they discovered I had a venous leak. That was the cause of my ED, and they told me that despite my youth, the best alternative was a penile prosthesis.

The surgery (05/02/2026)


Knowing what had to be done, I looked for a top surgeon here in Spain and financed the implant surgery. The doctor’s choice was the 22 cm Coloplast Titan One Touch. He informed me of the pros and cons of the procedure, but I decided it was the best option if I wanted to have sex starting now, rather than spending decades on pills and injections while constantly worrying about the rigidity of my erections.

The surgery went smoothly; I spent one night in the hospital, and the next day I went to a hotel to rest during the initial post-op days. Naturally, during those days I was bandaged up, had limited mobility, and was in pain and medicated, but there were no major complications.

After the surgery

After a few days at the hotel near the hospital, I was able to return home. Following my doctor's orders, I spent the first three weeks with the prosthesis inflated; it was truly uncomfortable for everything, but that’s what the surgeon required. On February 27th, the doctor deflated the prosthesis for the first time and taught me how to perform the inflation and deflation cycle. Since then, at least once a day, I’ve been doing the cycle: inflating the prosthesis, leaving the cylinders full for 30 minutes, and then deflating.

How I feel now: The erections are perfect. The penis size is practically the same as before the surgery (keeping in mind that I used to be a 'grower' and now I'm a 'shower,' and that it’s now a bit less 'round' and a bit 'flatter,' but I don't mind at all). Regarding sex, since I still feel some minor discomfort, I haven't wanted to try penetration yet, but I have masturbated and received oral sex, and overall, it’s been very good.

The issue is the flaccid state after deflation. Today marks two months since the surgery and just over a month since I started the cycling. What is the problem? Once I deflate, the penis stays flat and bends easily, but after a few minutes, the cylinders partially refill until the penis regains rigidity and size. It’s one thing to have a larger flaccid penis than before (which I expected, though maybe not reaching past the testicle line), but it’s another thing to maintain the rigidity of a semi-erection.

It is really uncomfortable to make any movement in this state. I can feel there is fluid inside, and it creates a 'lever effect,' bending awkwardly when I lean over to pick things up, climb stairs, or sit in a chair. My doctor tells me that the Coloplast Titan is very rigid and that it takes time to 'tame' the material so it hangs more naturally. He says the internal scarring of both the reservoir and the cylinders contributes to this persistent rigidity. He tells me 'time and patience,' but what does that actually mean? How much time?

I am a train mechanic—a job that requires a lot of physical effort and constant movement—and right now, it is extremely difficult to work like this (I don't care if people see a huge bulge in my crotch; I’m talking about daily comfort).

I’m attaching an image of how the 'flaccid' penis looks after an aggressive double-deflation and after walking naked around the house for 15 minutes to let gravity help point it downward. As you will see, although it doesn't look erect, it looks turgid and rigid. Right now, I don't know what to expect. Will I always have this turgid, rigid, and uncomfortable flaccid penis? Am I doing something wrong with the deflation technique? If the situation is supposed to improve, what should I be feeling or seeing, and in what timeframe?

Titan offers a “One Touch Release” and a “Touch” but nothing called “One Touch.” Not nitpicking it will just help understand which one you have. The touch is the newest though they all have pros and cons.

Looks great brother, fill out your signature.
Have you tried a complete squeeze your dick deflation,Then immediately give your pump one solid "pump" to lock the valve?
auto inflate is a common problem for most of us and an issue for all the manufacturers. there devices are made to last years so they have to make em tough. I think auto inflation is the byproduct of building then tough. because I have a dog ear on my left cylinder I typically add three short pumps for my work day and it doesn't increase by its self. give it a try, good luck.

Thank you for the detailed journey. I'm nearing 28 as well and am close to deciding to get the implant. I'm sorry about that inflated flaccid state. Maybe underwear that squishes tightly will force the saline out of the cylinders and stop it filling up?

Do you also have a picture of the penis in erection? I'm always looking for examples of what I might be working with after the implantation.
