Getting Revision Surgery with Dr Kramer in November (2025)

Hi All.

I had original surgery with Dr Kramer in DEC 2018 with an AMS CX 23cm implant. It failed in FEB 2025 after some rough sex and I've only been able to pump it up partially before it goes flat around 4-6 pumps. Bummer, as I was expecting this thing to last 10+ years. But it seems that men who are having sex very often are having higher failure rates than someone not having that much sex.

I decided to go with Kramer again with his experience in some special circumstances when implanting such as (silicone injections in ballsack for size), a lot of friends have used him also, and given that he is considered high volume.

The biggest downside is that I am in CA and have to fly there and back causing additional expense. He is currently in CapeCod and when I met him for the original surgery he was in Baltimore.

I'm now 58 yo gay male in very good shape.

I am really hoping that this revision will be an easier process than the original surgery (mainly the swelling in the ballsack).

Any thoughts, questions, or comments would be appreciated.

good luck - i've had a few revision surgeries but the 1st revision (after the initial implant) was the worst for me. surgeon had to do a lot of work to undo issues from the initial install so was much more swelling, bruising, and soreness. Took almost 6 weeks before I was back in the saddle again.

Subsequently, have not had any issues at all - revisions were straightforward. Was cleared for sexual activity within 2 weeks.

The travel back-forth is a big consideration for you, I think if there's any issues its nice to have surgeon close by within driving distance. You're more likely to have issues with revision surgery since it is more complicated.

I am in a similar situation. I am a 72 year old gay male who had a CX in 2015 when i was married to a woman. I came out in 2021 and am divorced but have a full time gay partner. My CX has not failed but it softens up after 15 minutes of heavy thrusting. Dr. Clavell in Houston is installing a Titan on 11/12/2025. I can have sex for a lot longer and can cum multiple times, but just want a rigid dick.

Are you still going to stick with a CX or will you switch over to a Titan? I'm wondering whether the CX dacron/silicon makes it more vulnerable to rupture with rough sex (this a a highly subjective term, one persons plain vanilla could be rough for another).

Dr. Kramer said he would just install the CX again. He feels that the Titan has a higher chance of infection than the AMS.

I didn't have any complications from the first surgery and I am getting re-implanted with the same model. Curious as to why you feel the revision surgery is more complicated?

Do you know if Dr. Kramer did standard dilation or corporal excavation when he cleared the corpora cavernosa? Either way, inserting the new cylinders should be a lot easier and less abrasive since you’ve already had the corporotomy and spent the last 7 years opening up those 2 tunnels (in theory).

Unfortunately there’s no way around that penoscrotal incision and he’ll have to add a new reservoir and pump since Boston Scientific calls for a full replacement after 3 years. It should definitely be easier since you know what to expect and are already familiar with recovery, the AMS system, and won't need to cycle.

23cm? Damn I'm jealous. You’re pretty maxed out, but maybe he can add longer RTE’s (up to 7.5) so you can cycle/stretch again and potentially add some length? Think of it as an upgrade vs. a replacement. I honestly can't wait for my 2nd surgery so I can hopefully add longer cylinders and be a candidate for the CX or the LGX (I currently have a CXR)

I travelled to the Cape and only saw Dr. Kramer once–the day of my surgery. He’s top-notch and worth the trip. And if you haven't been to Provincetown definitely try to visit before the surgery while you're there.
