Penoscrotal Vs Infrapubic

I'm looking at getting an implant very soon. A few implant patients have advised me to go for the penoscrotal approach if I can.

Apparently infrapubic can damage nerves and leaves a much more annoying scar.

I've been consulting with Dr Rees. I'm due to hear back from him this week. As far as I know he does infrapubic. I'm not sure if he does penoscrotal, I'm hoping he does.

I've noticed a lot of surgeons are doing infrapubic these days, for example Dr Modgil who is now one of the most esteemed implant surgeons in Europe.

What do you guys think? What would you do if you were me?

If you haven’t already done so take a couple hours to scan through the posts on this subject. Searching the word “infrapubic” has over 2200 hits. “Penoscrotal” has over 1600. It’s a topic that gets discussed here a lot.

My takeaway from those discussions over the last few years is that both methods are valid and both methods can get great results.

I personally had a preference for penoscrotal. Avoiding the tiny chance of nerve damage was important to me. And my surgeon used a vertical incision on top of a natural seam on the scrotum so within days it was invisible.

But as I said, both methods are valid.

After you have read through everything and feel like you have educated yourself about it, have a discussion with your surgeon about why they prefer the method they use.

Here is the tough part; If I had a strong preference for a method different than what the surgeon prefers I would NOT ask that surgeon to do the other method. I would search for a great surgeon that does the method I prefer.

I think it’s a BAD idea to try to get the surgeon to do a method they are less comfortable with. To get the best result you want the surgeon super comfortable, relaxed, and at ease, doing the method they have done hundreds of times before.

A different method will likely involve a slightly different set of surgical instruments. A subtle but important distinction. Because not only is the surgeon on less familiar ground, but the entire OR staff are on less familiar ground as well.

Find the best surgeon you can access for this and then put yourself in their care. Do everything they ask you to do to the letter.

Hi, thanks for sharing this. I'm waiting for a response from my uros secretary now. He hasn't responded in 2 days which is odd. I might need to go with a different uro now if he doesn't do penoscrotal.

I’ve had both infrapubic and penoscrotal without issue.

Do you find infrapubic reduces the depth of your penetrative length? Like - do you feel the tubing popping out of your fat pad? Is it a bit like wearing a cock ring on the base of your cock but behind the skin?

I’m not sure if the infrapubic method impacted my size at all. I was shorter than pre-op but I only had the implant for 3 months before getting a bigger implant put in.

In terms of feel, it felt pretty normal. Definitely didn’t feel like I was wearing a cock ring.

So does the base of your penis feel OK since you've done infrapubic?

Or can you feel the tubing in your fat pad if you press into your fat pad?

I think the primary issues with IP are cosmetic. A more visible scar, more visible tubing. The advantage is a faster recovery. A highly competent surgeon won’t get near the nerve bundle.

My surgeon (Dr. Hakky) initially planned an IP procedure for me. I’m married and he figured the easier recovery was more important than the minor issues of more visible components.

On my pre-surgical workup, I told him that I was getting divorced. He immediately pivoted and said we should probably do a PS approach, as keeping things less visible might be important when I have new partners.

I appreciated this — a good surgeon will tailor the treatment to the patient and their circumstances, instead of a one size fits all mindset.

I think either approach is fine, just talk it through with your surgeon and make a decision together based on what you want and need.
