Small scrotum, big pump - help?

Postby AtxGuy » Wed Jan 11, 2023 7:31 pm

I’m two weeks post-op with an AMS pump on right side of my scrotum. Pump block is positioned tubes-down, deflate button pointing behind. Right testicle is sitting on top of pump block. Pump bulb is wedged between right testicle and midline of scrotum. It’s hard for me to squeeze bulb without getting a ball too.
At surgeon checkup today he said “not ideal placement, but you’ve got a small scrotum. Over time it’ll probably drop a little bit”

Does anyone have a similar experience? How did it turn out? Anything I can do to stretch scrotum? (I wish I had thought about stretching scrotum for those two months I was doing VED preop)

AtxGuy,
I don't know if this will help at all. I don't have a particularly small or large scrotum or testicles, but what I can tell you is that you need to give it time. There is probably still swelling, and that affects the pump placement. My pump, also AMS on the right side is nowhere near where it was post-surgery. Now, the pump moves around, depending on how tight or relaxed my sac is. You will be ok.

It is not easy for a surgeon to ensure the location and orientation of the pump. Some surgeons go so far as to stitch it in place, but even that is not a perfect solution.

My left testicle for years was virtually hiding up above the pump.

I would be more concerned about the (as I read your post) that your pump is upside down. I would try to get it right-side up if I were you (with the advice and consent of your surgeon, of course).

Skin is stretchy. I would not be concerned about that. Over time it will probably stretch to accommodate your extra contents there.

You will develop good technique for pumping. It will take time, but you will figure it out.

Atxguy, did your surgeon mention gently pulling you pump down a few times a day? This is a common suggestion from many surgeons. Search your memories & perhaps check with your surgeon or his staff for his at home suggestions.

No, but the tubes are already down to bottom of my sack. There’s just no room for pump in bottom so it’s upside down with bulb-up, deflation block down.

I think my issue is more “10 pounds in a 5 pound sack” more than anything

Yes, your pump is upside-down. It will be easier to squeeze the pump bulb if it is at the bottom of yous scrotal sack. You may be able to gently, over time get it turned around. At the same time the manipulations may stretch the skin.

Slow, steady constant tension on human skin will stretch it. Plastic surgeons sometimes can get extra skin for grafts (not that I am suggesting a graft for you) by injecting silicone under the skin (I recall reading about this years ago).

I suspect that if you get the pump right-side up the lack of scrotal volume will be less of an issue.

I'm 4 weeks post op and have simar issue with placement. Mine is pump pointing forward and VERY prominent. I'm currently just trying to give it movment while having a warm bath as everything loosens up in the bath. Just need to be patient as it's so soon after the op and there is swelling. I noticed a difference qhen I hit the 3 and half week mark.
