I was implanted about 6 weeks ago, and am extraordinarily happy with my implant and, like many others, my only regret is that I did not do it sooner.
My question has to do with uneven placement of cylinders in the glans. I can feel that the right cylinder is about 1/4 inch higher than the left.
When flaccid, I can see no difference, and when fully engorged I see--and feel--no difference. The glans becomes engorged as pre-surgery. My partner has said that she doesn't notice anything amiss during oral.
Am wondering about how common this is, and whether others who have uneven placement ever experience any problems either in the near-term or longer term.

During surgery the doctor measures both sides. This is because there can be a difference in some men. RTEs are available in .5 cm increments. So many doctors will advise they will get the cylinder tips in the Glans within .5cm of each other. A few doctors will remove materiel from the RTE end of the cylinder to get the tips in the Glans perfect. A 1/4 inch is .635 cm so a .5 RTE maybe could have been used but you are in the ballpark for most doctors. Another thing is that you are just 6 weeks into recovery. The Titan does expand a little bit. About .75 cm for the 22cm Titan so this may even self-correct over time. So go have fun with it. A great reason to use it often to break it in and see if the length will change. Man is this great.

One of my tips is a little further into the glands than the other about a quarter inch. I discussed with my surgeon and he stated that it was not a worry as long as I didn’t have pain. He told me that in the future in a revision he would add a rear tip extender to make them even.

It is cause there may not be enough space for both tips to reside in the narrower part of glans?

I would suggest to wait for 6 months. It seems like it takes about that long for everything to settle in & many small problems seem to resolve theirselves. Just guessing, I bet your doctor won't address it before 6 months either.
I've speculated before that a RTE can be added in what I think might be a fairly easy revision. But I have to add that I don't remember any member saying that he got just a RTE revision. Adding or subtracting a .5cm or more.

Why did this change with the third implant? The Dr could've just reused the space instead of drilling further right?

It was because of the infection I had where they took the implant completely out for 6 months and then reinstalled the implant after the infection was gone. I used the vED everyday for the 6 months which helped them reimplant me but I did have some residual scar tissue. I actually have had 7 surgeries related to the implant. Three related to the infection. So I am thankful they were able to reinstall the implant at all. You can look at my previous posts for the details of my implant issues.

that you went through all this the initial implantation by whom was it done? just to understand, I see that systems made by the best in the world almost never have problems in the first installation

Yea I believe if I would have used dr Doyle and Boyd in the first surgery the outcome would be different and I could have prevented all the other procedures and infections. I stuck with the dr for so long because he had great bedside manner and I thought it was my body causing the issues. My dr information is in my signature.
