can RTEs cause protrusion of tubing?

I consulted another top urologist in India about my protruding tubing.
He said it could be because of the 4 RTE. The surgeon ought to have used 18 +1 RTE. Instead he has put 15 + 4 RTE.

He suggested 2 options:
Get the same surgeon to put 15 + 4 RTE
OR
ask him to perform a minor procedure to pull the pump down.

I definitely need to solve this problem of protruding tubing and am inclined towards the latter.
