Sorry to dig up an old thread, but I thought I'd chime in with my experience.
Like Kodixx, my pump was also placed very low in my scrotum, basically resting at the bottom, so there was/is no room to pull it down. My urologist didn't give any specific instructions on pulling on the pump, but I've been working the pump around during my recovery to make sure that it doesn't adhere to any neighboring tissue inside the scrotum - especially the testicles! (yikes, @DaveIsStillAlive... that sounded painful!)
Anyhow, about the forming of a pouch in the Dartos fascia: my post-op journal has a detailed description by my urologist on how they formed a pouch with a "blunt object" (probably just using their finger) for the pump in the Dartos fascia, and after placing the pump, the pouch was sewn shut with something called a "tobacco pouch stitch" (there's probably a proper English/Latin translation for that). So they indeed seem to do this, like wanttokno mentioned.

Nordicguy, thanks for adding your thoughts and experience to this thread. Yes for awhile my Dr thought what I was experiencing was the pump being held in place by the pouch. It wasn't until an office visit that he confirmed the pump bulb was really adhered. What you relayed is very good advice -- that even if you don't need to pull down on the pump, be sure to move it around as much as possible/reasonable to ensure it doesn't get adhered to something.
