I'd try injectable temporary filler for girth enhancement to see if that fixes your issue to your satisfaction, before going under the knife again while you still have a working implant. A number of doctors do the filler injections, however they are mostly plastic surgeons and don't have experience with penile implants, rather I'm sure most of their patients do not have implants.
BUT there is one doctor who offers the filler injections who is very experienced with implants... Dr Hakky. He injects the filler in your shaft and it will stay there for roughly 1-1.5 years. The cost is $5,000 and you can expect to gain about 0.5" girth. It is easily reversible if you don't like the look. There are more permanent filler products that you can research on other forums, but this would be a good start to see if you are satisfied with the shape of your dick after the girth enhancement filler.

Thanks Merrix so much for your in-depth response. It certainly gives me something to think about. I guess I do need to really weigh this all up but your analysis that the Titan will push cyclinders closer together is exactly what the Urologist told me.

I'm thinking aloud here and may be way off the mark, but I wonder if its something to do with the surgeon cutting the cyclinder holes too large so that there is very little tissue left and the expanded width of the cylinders is literally the entire width of the whole penis? I do feel the cylinders are very hard through the outer skin.
I went and saw the most prominent implant surgeon in my country who told me that over time with a Titan, he believed the width of the cylinders would expand and they'd push together in the centre more. He said he thought this would probably remove some of the flatness and give me back some girth. He said the Titan width was only limited by the tissue in the penis where as the LGX doesn't expand more than its max width. Finally he said he'd also be able to instal the tubing further into my body (by not using RTE's but longer cylinders) so some of the existing tubing that is slightly jutting out isn't hurting during sex from impact with it.
I accept a lot of the feedback in this thread is that I may not derive as much as a benefit as I'm hoping to. I'm grateful for all your advice and would like to receive as much advice from you guys as I can. You guys are right that I need to carefully consider whether its worth going under the knife and post op complication risk if there is little benefit.

SuperReverb - I was reading your December 16 post about the dimensions of your erect penis. You mentioned it being 1" flaccid but expanding to 4.5" inches wide when inflated. Am I reading that correctly? That sounds incredibly wide.

I was implanted with a titan 9/21. I had horrible atrophy (shrinkage) permanently that I can promise after titan, my flaccid dick sometimes gets in the way. I gained girth and obvious hardness with titan and if I do say so myself a natural and pretty good looking circumcised dick now both flaccid and erect. My wife says that before she was able to take it all when doing oral and now she can not. She loves the extra girth on penetration and she’s 100% satisfied with the implant and I am as well. Much more important than looks and all my ego was severely bruised prior to the implant. My mental health now is great. I’m glad I get to satisfy my wife again. Good luck with your revision.

Hitting just under 7. So only a .75 increase from flaccid. All in all, I don’t mind it, but I do have to be careful what pants I wear and where. No khakis for me. Gray sweatpants only to be worn with caution. I’m still a little pissed about losing length, but I think that was from years of ED and not the implant itself. I believe my doc did everything possible to maximize what the was working with.

I have a titan otr, 24cm. My dick is noticeably flatter (to me) compared to before ED. That being said, my partners have never noticed or said anything.
Titan OTR. Dr. Hakky - successful surgery and very happy with outcome.

I would think that if flatness is a problem with an LGX, the Titan would be even wider and probably as flat.
With a CX, I have around 5 1/2" flaccid, but I don't have a problem with it showing. I wonder (but don't remember) how it looks in a swimsuit. I've only gone out in my own pool with it.
