I think this is because Medicare is generally aged based and IPP surgery is seen as a frequent medical necessity.

Also one thing I've learned you might not be doing is there's a great way to make this all much easier - just call them and ask. Seriously. You only deal with professionals who are motivated to help you. Whatever has stopped you from calling them wasn't a real concern.
