I've been implanted for about 6 months.
I think everyone is saying much the same thing but maybe using slightly different words.
I'll try and break it down as I understand it.
1. Arousal and engorgement are not the same thing but are closely related.
Arousal is the psychological process of sexual excitement that eventually increases blood flow to the sex organs; i.e. it's more than just physical.
For a guy engorgement is the purely physical increase of blood flow to the shaft and glans of the penis, which causes an erection (without ED).
So arousal causes engorgement.
2. Arousal is required for glans engorgement after implantation. It has a separate blood supply to the shaft (corpus cavernosum).
The implant will not inflate/engorge your glans (aka head, knob, fireman's helmet, or other names that I've forgotten).
3. Shaft engorgement after implantation is provided by the implant cylinders.
4. If lucky, after implantation you may still get some natural engorgement of the glans and shaft, or through use of Viagra, Cialis etc.
5. With an inflatable penile implant (IPP) or a malleabale (MPP) you DO NOT need to be aroused to have penetrative sex.
Getting an erection is now a mechanical procedure but you can make it part of foreplay if desired.
6. The implant is simply a hydraulic device that when pumped up gives you an artificially erect penis.
7. You control the degree of firmness by the number of pumps that you apply. I can get my dick extremely hard.
8. While you can have sex with an implant without being aroused, it will probably be less enjoyable for you.
As Chuck said earlier you may still become aroused just through the physical act itself. Everyone is different.
Whether a female partner will enjoy sex less without glans engorgement is a difficult question to answer.
I've had no complaints, but I only have one partner.
That was way more complex that I intended but hope it helps.
