Penile scrotal VS Infrapubic?

The old take on this in the posts on this board is that penoscrotal was better for pump placement and infrapubic better for reservoir placement. But that depends on the doctor you use. A very good high volume doctor will be good no matter which approach they use. If you are using a low volume doctor then this is more of an issue. There is also a nerve bundle that needs to be avoided with infrapubic that is not a concern with penoscrotal. Again only an issue with to be concerned with lower volume doctors. And with infrapubic you can have 1, 2, or 3 incisions. Depending on if a drain is used or not. These are visible if you trim or shave. They take a bit more time to heal and become a bit less visible. Penoscrotal if done with a vertical incision will completely disappear. And even if a horizontal incision will become virtually undetectable. I have had 3 scrotal surgeries. A Hydrocele, implant with scrotoplasty, and a repair. Only the implant with scrotoplasty was done with a vertical incision and you cannot see any of the scars. As was said the tubing from the cylinders to the pump and reservoir are more noticeable with infrapubic in addition to the more visible scars. You can search for the thread "implant pictures" on this board to see the infrapubic scars. And it used to be widely accepted that penoscrotal gave better access for revisions and repairs. But some doctors are doing really amazing stuff with infrapubic now. In the past some have said due to the location there was a bit higher infection risk with penoscrotal. But that is very very slight if at all and strict wound care is used. Just easier to keep infrapubic incision clean and dry. Some information was put out that infrapubic was better for measuring cylinder length. Again all this is only a concern with a low volume doctor. The really good doctors most of the time it will not be a concern. That said no one is perfect and even the best with have a problem but will have them less often. And there will be less scrotal swelling in most cases with infrapubic. I had very little however with my penoscrotal surgeries. And that may or may not effect activation time.

So it depends on you and your doctor. And how you feel about the tubing and visible scars. And how good the doctor is for placing the pump (infrapubic) or reservoir (penoscrotal).

1. OK, I still don't have a clear understanding of the difference between the two.

A:Infra pubic: 1 incision above the penis where they stuff all the plumbing through, the resevior goes in the vicinity of the belly button.

B: Penile Scrotal: 1 incision in scrotum all the plumbing goes in through there,

Does this sound right?

2. Pros and cons of each?
3. Which do you have, hows it working for ya?

i've had both types. Pros and cons for each. Infrapubic. Eliminates distal crossover. That is what I have and it causes issues. Also you can use VED almost immediately after surgery. But typically the doctor measures you a little shorter. also more of a chance for pronounced dog ears, ie tubes bulging under your skin. easier to put your reservoir in a good spot. Penoscrotal. better measurement if you have the right doc. My guys subtracted the expected expansion of my LGX from my actual length. Gave me about 4 inches the rest I had to fight for. The pump is easier placed if you have the right doc. Mine screwed that up using a preconnected unit that left no extra tubing for movement. I had to have another surgery to fix that. placing the reservoir can be a challenge.
Overall let your doc do what he does best. the differences in my opinion are a wash. bad doc, bad outcome. good doc, great outcome

I had penile scrotal method and also the surgeon did a scrotoplasty at the same time. I had very little bruising or swelling and now almost two years later, no scar is visible on the scrotum. For men who remove all their pubic hair the scar from the infraPubic procedure might be visible when nude or in a swim brief. I do not have a fat pad in that area and wear some low cut swim briefs so opted for the penile scrotal and scrotoplasty. My surgeon indicated that as surgeries go, either way is a pretty simple p, straight forward procedure for the surgeon.

I think I missed answering the actual question. So let me try again. Penoscrotal. Typically surgeons make a 1 1/2 in vertical cut below your penis about the center of your scrotum. My docs made a 3 1/2 in horizontal cut at the base of my penis all the way across my scrotum. I've never heard of that before or since. Infrapubic. A 1 1/2 inch horizontal cut is made on top of your penis at the base in the crease of skin just below your fat pad. This heals fast and leaves no scar. My docs made a 2 inch vertical cut above my penis right across the center of my very fat fat pad. Not only did they stitch it but they had to staple it also. Needless to say healing was slow and I needed an extra visit to remove staples. Now going on 4 years later it still constantly itches. Fat does not heal.
Your IPP is different for these 2 procedures. Penoscrotal the tubes coming out of your inner penis are on the side and pointing down. Infrapubic the tubes come out on top and pointing up. This can cause larger dog ears when the tubes bulge and bend just beneath the skin. Penoscrotal the docs basically measure from underneath your penis and hold it more upward like a normal erection. This usualy gives you a little more length. Infrapubic the docs measure from the top and so point your penis more downward. So a bit shorter stretch.

I had the Infrapubic type implant, had a drain tube and after 4 years, have no visible scars unless you really look closely. I do have a fat pad but even pulling that back I can't see any scars. So I guess it just depends on the surgeon,and how well you heal. If you look real close you can see a slight scar, but you have to look.

I had a 3/4 inch vertical cut with infra pubic operation. The tubing running around was concerning for about 8 months. Dont even notice it now.

Actually regardless of what method you use the reservoir goes up into the abdominal area in between the abdomen and the skin mine is on the left hand side I only know where it is because right after surgery there was a little patch of yellow
