I had a similar reasoning - there are risks and complications. However, I did as much research as possible, and one of the top surgeons in the world as far as implants is concerned is doing this. So basically everything that was in my hands was done in the best way possible, the rest is in the hands of the surgeon.

If you were in the hands of an unknown surgeon, there'd be a lot to be concerned about, but under the circumstances you're in the probability for any serious complication is extremely minimal.

My own post gave me an idea. I loaded google chrome. Though I could not read but a a small bit on Chrome, I was able to print it. Thanks. Been looking forward to reading it. Also reminded that Chrome works even though nothing is easy to use on it. Thanks. d

Hi everyone,

I'm in the process of booking my procedure with Dr. Eid. Question for you - there is a medical clearance form that is required. Essentially, it is meant to inform my doctor that the implant will be inserted and it asks that the doctor agrees to the plan. If so and in order to proceed, a comprehensive medical note which discusses the patient's current medical issues particularly focused on cardiovascular health is requested by the anesthesia staff. The patient's pre-op requirements and written permission to proceed are also asked for.

My big question is which doctor needs to fill this out. I have a family doctor that I have not seen in years. the doctor is also very closed-minded and would not even understand the need for this surgery or what it is all about. Separately, I have been going to an ED clinic for the past couple years. They have doctors there which prescribed me Cialis and the injections and previously ran some blood tests, etc. Would it be appropriate / acceptable for me to ask one of those doctors for a note? i think they would be way better positioned b/c they actually understand the concept of an implant and are closer to my situation with the pills and injections. The only problem is they are not my general practicioner and so I don't know if they would even agree to write a release note.

Sorry guys,

Just a quick update on this....it had been so long since I visited my former family doctor....but I just spoke to my parents and it turns out he is now retired! So as it stands, I actually don't even have a primary care physician right now.

I'm a little confused about who will provide this clearance form now :? :?

I think your best bet would be to call Dr. EId's office and explain the situation to them and see what they tell you do, you may need to get a new family doctor, or he may allow your urologist to fill out the form, but that is what I would do. Best of luck

The medical clearance form typically if not always comes from a primary care doctor who is a medical not surgical specialist who will do a physical exam, take a history and order the preop labs needed by Dr Eid. The primary care doc makes sure that you dont have any medical issues that would make your surgery inadvisable or risky. The doc will evaluate your heart and lungs etc and confirm that your blood count, liver, kidney functions and clotting chemicals and times are all normal. Urologists at least any I have worked with dont do these medical preop clearances. Dr Eid might have someone he knows but that would involve two trips to NYC. How long would it take to get innto a new doc closer to home? Good luck.
