I can hardly say it is a patient's preference. Every surgeon has his own approach to perform this kind of surgery and excels on it. If you select a surgeon who mostly uses infrapubic, you may not ask him to do it with the penoscrotal concept.
The scar is more visible with infrapubic, but less healing time compared to penoscrotal. Also, challenges with the tubing and properly locating the pump.

My Dr. said this about infrapubic: "great for the surgeon (easier and faster), much worse for the patient". He considers it the lazy Drs way to do the implant.

every doctor boasts his approach, mine (Antonini ITALIA) has learned from an expert, he says that the infrapubic is very fast and reduces the probability of infection, plus the scrotum is a delicate point for a probable infection, everyone says that his is the best, that's why we are there to tell and judge the work that has been done to us

The skill of the surgeon matters way more than the mode of installation. As Dr. Perito says, a penis is a penis is a penis.
Born 1982. Implanted with Coloplast Titan 22cm in August 2021 by the great Dr. Perito in Miami. 6.125" BPEL, 5.25" EG. "It's the girth that gets her off."

My issues with the first and second implant wasn't the procedure, it was a (imo) less than skilled surgeon. My Dr for the 3rd implant is a genius and had to resolve the first Dr's errors.
