New TriMix user, have questions

New member and new injector, LOL
I am 57, was diagnosed as Type 2 diabetic over a year ago. Now controlled with Januvia and Metformin, last A1C test was 6.5, so good there.
I have blood pressure of a 20 year old, average of 127/70.
But slowly started having ED issues over a year ago with accompany of feet numbness and tingling, and 200mg of Viagra was like taking skittles, did absolutely nothing.
Had GP refer me to uro.
The uro was a waste of time. He never even touched me i never got undressed, in fact never got out of my chair once in the room with him.
No tests, finger pricks or anything. Told him my problem, let him look at my last blood test results and he wrote me a prescription for TriMix (Std). I assume he is just a script writer, not a decent doctor. He told me if I get to 40 units call him and he will up the strength.
Took the prescription to Walmart, Walgreens, CVS, no one had it. Called the uro office and they said "oh, we should have told you it is custom mixed, we recommend this local pharmacy"
They charge $99.95 for 4.5 ml of it. So it is kind of pricey, but i have the paper prescription, so I can go anywhere when I get it figured out.
It is Papaverine hcl 75mg, Phentolamine Mesylate 2.5 mg, Alprostadil 25 mcg. Those numbers sound a little high to my, but they call it "standard" (STD).
So after direction from pharmacy, was told to start at 10 units, increasing by 5 units until it worked, not to exceed 40 units using a .5ml 50 unit syringe.
10 units worked about 65% hardness, lost chubby after orgasm, maybe 20 minutes. 3 days later 15 units worked maybe 75%, again lost chubby after orgasm, again maybe 25 minutes. Tonight was 20 units, maybe 80-90% when hard, but chubby went away when it wasn't being paid attention to but would come back with stimulation, lost chubby after orgasm, maybe 35 minutes.
Now for the problem.
I have noticed when I orgasm, I get a splitting headache, like a migrane, right behind my temple. It starts getting real bad seconds before I orgasm and then during it gets almost unbearable, almost LOL, then after I orgasm it SLOWLY starts to dissipate, and usually after 30 minutes it is almost gone. But I can tell if i turn my head or shake my head, the pain will let me know it is still there.
What ingredient could be causing the headache?
I am satisfied with the performance of the injections, I am def hard enough for penetration and have orgasms just fine, but the headaches are a killer, and a HUGE distraction from enjoying the pleasures of the orgasm.
Any Ideas on the head splitting headaches during orgasm?

I've never experienced anything like a headache from using Trimix.
59. ED from prostate removal surgery in 2018. Low doses of Trimix worked fine but usually gave a painful erection. Sildenafil - hit and miss but caused headaches for 8-12 hours. AMS-700CX 21 cm cylinder placed on 2-22-22.

I think this is primarily a question for a good doctor. It doesn't sound like you have a good one. At least not a good Uro. However your GP might be able to point you in the correct direction. I know nothing of the topic other than i have experienced it to a lesser degree in the past. For me it was a temporary condition and was during a time of higher stress but doesn't sound as acute as what you're experiencing.

The main question is - did you have this kind of things before you started trimix or not? Do you also get it when masturbating or otherwise orgasming without using trimix?
I've been using EDEX, which is only 1 of the 3 ingredients you have, but it's never ever caused headaches or any side effects other than the occasional soreness in the dick.

In any case, as the others have said, you better check with another doctor, a SPLITTING headache can never be a good thing. Sadly, it can actually be a very bad thing, if it comes when you orgasm, but i don't want to scare you with things that only a doctor can verify.
39 years old, married. ED all my life because of spinal cord injury caused by a tumor in early infant age. Using standard EDEX20 since 2007. Increasingly bad results with EDEX in the last few years, but had very good results for at least 10 years.

I get a slight headache sometimes. I chalked it up to the mental stress of poking Li'l Alfa and angst. However, I don't have extenuating medical conditions or medications as you so I'd check in with a professional to see if there might be some sort of interaction with the meds. I've gone through 2 Uros. mostly due to shuffling insurance. The 1st. one seemed a bit aloof and gave zero injection training but at least inspected my junk. I too had the blank looks at the local CVS, found a compounding pharmacy only to find out that I needed to find a 'sterile' compounding pharmacy. At least you're getting some wood. I'm on the strongest mix the pharmacy makes and apparently have a venous leak so bad that I have to be on my feet to barely penetrate. I may have to mail order rocket fuel strength if the pharmacy wont make it. Talk to your doctor that's why he makes the big bucks.

I was on Edex quite a few years...enough to lead to scar tissue forming in my penis. That was an important reason why the VA basically talked me into getting an implant. Contributing to the scarring was the fact that since a 2001 DVT and Pulmonary Embolism, I've been on coumadin (recently changed by the VA to Xarelto). I know many guys are able to use Edex or tri mix long term without any scarring. I rarely have headaches though and did not ever have one with sexual activity, with or without Edex. As mentioned already though, just in case it is something with those cranial vessels it would probably be worth having that checked out. Or, if you had no such headaches before starting to use the tri mix, maybe it is time for an implant for you. IIRC private insurance generally insists that you go through each of the escalating steps of ED treatments, the injections being the last step before an implant, so if they are giving trouble, it could get approved on that basis.
Surgery at VA July 2017 after many years struggling with pills, shots, pump, etc. Should've done it way sooner! October 2020: VA just replaced the first one with another due to stuck valve...AMS 700 LGX.

Thank you all for the info. I was just wondering if it was a known reaction to the Trimix.
I did research the 3 ingredients side effects and only the Papaverine hcl mentioned headaches, but it sounded like a normal headache, not like I am having.
Thank you for the links bldoink, The Mayo article was especially informative.
I guess I will need to have a chat with my GP and see what he comes up with.
Knowing him, he will just say take 2 aleve 1 hour before sex, LOL
It is hard to find a good doctor in a small town.
