Venous leak confirmed. What next?

Hello gentlemen,

I got my doppler ultrasound done sometime back, and Dr. Garber had confirmed to me that I have a venous leak. I have access to the results now, and I wanted to compare it with others. In spite of the leak, Dr. Garber told me that I should try all options and not rush into the implant given my relatively young age. I've been lurking on this site for a year or two now and have gone through a lot of posts related to venous leaks. In a lot of these posts members have pointed out that implant might be the only option for leak patients, so it got me wondering why the Dr. Garber didn't directly suggest the implant.

I also came across posts where members mentioned the % of their leaks (example: "Dr. Eid told me that Implant is my best option since I had a 45% leak") - I don't have a such a %age number. All I have are the numbers below. I have tried pills and they are only good for oral sex, as as soon as I try to penetrate I lose my erection (I start losing an erection when I move around a little). My avg. peak systolic velocity number is 28.84, end-diastolic velocity is 7.56 and resistance index is 0.73. Is implant the only option for me? Any thoughts?
33 y/o, suffering from ED as long as I can remember. Never had random boners. Pills worked for a few years, but now losing efficacy. Venous leak confirmed by Dr. Garber. Likely going the implant route at some point.

Before my test I watched a YouTube video on the procedure. The Doctor explained that a reading of 6 to 9 on either side was chronic that 4 was the highest to have a sustained erection. Now this was just one Doctor. As I have found out these guys are pretty much free agents and make the rules as they go.I had 6 on both sides. That was more than enough to convince me that waiting around would only prolong the agony. You have over 7. But you need to come to peace with your choice. I did with mine took about 2 seconds. May take you longer.

Hi, I am 59 years old and I recently had my implant surgery 4 weeks ago. I've been having problems about 3 years ago. Before the pills ( Viagra, Cialis) worked perfectly, but then I started to have a erection and in a few minutes my penis became flacid, I believe it was a result of my diabetes, nothing was working. I became very angry and frustrated. I use to enjoy sex a lot and very soon I'll do it again. For me the procedure was a breeze. Don't waste more time, you are young and need to enjoy life. So go ahead and talk to your doctor. Best wishes!
59 Years old pills do not work anymore, Tried Tri Mix not work, 2 years with ED.

It is hard for a lot of us in our 50s to relate to someone your age. A lot of us have already had a long and good sex life then something happened and it alll went bad. For me it took over 2 years for me to find my 4th urologist surgeon to do adequste tests to tell me i have blood going out as fast. I also developed peyronies probably from trying to have sex with s semi hard dick. That is something you do not want to happen. That is a whole new set of horrible problems. If my first doc had told me i was leaking i would have had an implant over 2.5 years ago. As it was, as soon as i found i had the leak i scheduled surgury as soon as i could.

Before peyronies i would have had my choice of either lgx or titan. After peyronies it was the consensus of 4 surgeons my only option was a titan. In addition you will over time lose elasticity. This equals lost length. While it might come back with cycling an implant it might not.

You have a lot to consider. I have had my titan 4.5 weeks now. I am still getting used to it for sure but already it is nice to have a hard dick on command like my old days. In my 50s no big deal about pumping it up. That is something you have to consider. But you also will have to decide if you want to keep trying pills and or trimix. One negative with trimix is the injections can cause peyronies. Back to a real negative.

I will say this.i discussed this whole situation with my sons this weekend as they were both in town. They are 30 and 38. I wanted them to know what happened to me and what i did to fix it. I wanted to make sure if they were ever faced with a similar situation there is a light at end of tunnel. I would recommend this to either of them if they ever needed it but also not necesarily wait till the absolute last minute only after everything else was tried.

Whatever you decide and that is the one sure thing. It is something you will decide after all options, i will wish you the best..
18 cm plus 1 rte titan installed March 2019. Revision March 2020 by Dr. Andrew Todd, Richmond KY. He replaced the titan with an AMS 700 LGX 18 cm cylinder plus 2 rte for 20 cm total length.

Have you tried Trimix injections? I used that for about a year before they stopped working. For some people they work for over a decade. Ask your Dr. to inject you in the office and see how it goes (do NOT go to one of those ED places). This may work for you until the next latest and greatest implant comes out. I would still be using the trimix had they continued to work. I love my implant now but it's not the same as the real thing (close but not quite the same).
Suffered with ED for most of my life. Viagra then to Trimix then implanted 3/8/2018 with AMX 700 CX 21cm with 1cm RTE. 50 years old Urolift procedure performed 30 day prior to my implant surgery.

I agree, I'm 80-85% convinced that implant is the way to go. But it is a life altering decision, since I'll need a handful of them in my lifetime, and committing to that decision is just taking some time.

Thanks for sharing your story SW, glad it worked out for you. To be honest, that is a major conundrum that I'm in right now. Logically, getting an implant now seems like the best option, given my severe leak and the side effects of the injections. From the trends that I've observed on this website, it seems like Trimix works for a few years and then it gradually loses efficacy, and during this time a lot of people develop peyronies, fibrosis/scar tissue or some other complication. These negative effects create a much more difficult scenario for the implant surgeon to deal and can actually affect my chances of a successful implant. I currently have zero scarring and plaques according to my ultrasound. I do have a 15-20' natural bend and I feel that I'm at a higher risk of peyronies because of that. I'm going to give Trimix a shot and see how effective it is. Doing this more so beacuse I don't want to regret no trying all options before the final option.

No, I have not tried Trimix. I was injected with 20mg of alprostadil and that did absolutely nothing for me, no effect at all. It was quite disheartening to see that and made me realize that Implant might be my only hope. But, as you have suggested, Trimix could be helpful. I was under the impression that if the leak is severe, (which it does seem like it is, per other members on this forum anything above 6 is quite severe) even Trimix might not help you. But I'm still ready to give it a shot. See my response to SW0110 above for my opinion about Trimix.

When the pills stopped working adequately I had Dr. Eid check me out. I had venous leak. I tried injections about 3 times and decided that while they worked, I didn't want to deal with them anymore.....so I went right for the implant without wasting time. I suggest doing the same thing unless the pills work for you......
Implanted Jan 4 2017 by Dr. Eid - 70 yo and single...ED gradually over 15 yrs...tired of pills, injections, cock rings....happy with my choice and results. Titan OTR - one 20cm cylinder - one 20cm+1 RTE...."got a rocket in my pocket"....

My only comment with pills and a venous leak is i am pretty sure i caused the peyronies trying to have sex with a viagra infused dick which was just not really hard enough with blood going out as well as coming in. Still just not hard enough.

It took me more than an average dose of trimix for an erection so just bypassed that. It also did not last very long.

That is why i went the implant route asap. For me and only me it was my only resl option.

Looking at my solid 6 inches. Feeling good about my decision. I might get more with time. It has been about 5 weeks and i am at almost my presurgery stretch length. Girth is also getting better. I am pumping up 2x day for about an hour each. For that hour i keep trying to get another pump every few minutes. Achy but i keep trying.
