Peno-scrotal incision = cut is underneath the penis.
I was told by my surgeon that peno-scrotal is better since the tubing exits the cylinders on the undersurface of your penile shaft -- therefore those tubes cannot be felt during lovemaking.
In contrast, I was told that the infra-pubic (above the penis) incision means that the tubes exit the cylinders along the top of the penile shaft; and there are some situations where the women might feel those tubes during love making.
A few months ago, I saw a picture of an implant where the bulges from the tubes could be seen along the top surface of the penis. This seemed like a not good situation to me since I could imaging that a woman might feel then during deep thrusting.
Similarly, some have stated that they are not comfortable with the cock ring because it constricts around the tubes .
For me, personally, my tubes enter from the underside of my penis, and the entry is way back, so that they absolutely cannot be felt during lovemaking and also I have no concerns about a cockring squeezing on the tubes.
Certainly, telling your surgeon how to do the operation (infrapubic versus peno-scrotal) is way out of line, but I have heard enough here on franktalk that there does indeed appear to be an issue with the infrapubic approach when it comes to being aware of the tubes.
