So an update from me:

My doppler is scheduled for May 3rd with a follow up with the consultant on May 17th. On the 17th I am planning on telling my consultant that I want to go ahead with the implant. Thanks to this forum, I have been able to answer a lot of my fears/questions and feel as though it is the right decision for me. To add to this, the tipping point was yesterday, when a hot girl who I have liked/been flirting with on and off for 6 months or so told me she had a free house on Friday and would I take a day off work to come and stay at hers. It was horrible having to turn something down which I want so badly, because I know I wouldn't be able to perform. Hopefully I can get the implant and revisit this situation!

Anyway, a few questions I plan on asking my consultant and please feel free to add anything which I may miss. Some of the questions are more so I get an idea of his approach as opposed to me wanting to know the specifics

1. How many times has he done this procedure? I want a high volume surgeon who has done this many times, not an amateur who does 1 or 2 a year. I am seeing a doctor at UCLH (London) called Dr Muneer but if he doesnt have the expertise I will ask if David Ralph who I have seen recommended on here can do it
2. Does he perform the stretch test or injection induced erection test to see which implant size to use
3. Does he use Coloplast Titan OTR or AMS (I don't really have a preference at this point in time but possibly leaning towards OTR)
4. Whether he uses infrapubic or scrotal incision
5. Whether my varicocele has any relation to the ED? Would he recommend scleropathy (i haven't read up much on this...)
6. What is his preferred method of placement for the pump...high and behind testicles?

Any input on questions to ask I would appreciate

Hi Star,

UCHL is a centre of excellence, Ralph and his team do more implants than anyone in the UK so your in good hands either way.

I'm told by a guy that was implanted by Ralph that you get a choice of implant. They will help you decide. Also they generally work together so you may get both on the day of surgery.

42 years old, Venous leak all my life. Pills worked but not so much then I foolishly did Scerothreapy (See young guys thread). Now totally impotent and just want an implant to stop me from completely crazy.

I take it you recommend whole heartedly not to have scleropathy!

Star,

Keep me updated. Im mostly likely going to schedule my surgery here in states in September. I have another minor surgery I need to take care of in May, and I will require a couple of months to heal before I can do another.

You asked some great questions, many of which I will be asking my surgeon as well.

If it matters I am going with a Titan. Rigidity matters a lot to me, and based on what I keep reading the Titan is better for this. Furthermore, Ive read numerous times that the Titan has a more inconspicuous pump, which Id prefer.

And lastly, I know exactly how you feel when you have to turn down a girl who presenting herself to you. It fucking sucks. I hope the discipline and self control I need to use when confronted with these situations will make me a better person in the future.

Thanks Cnidium. I will probably continuing posting on this thread my doppler results and next consultation.......once I have got a surgery date I will probably do a journal from time of operation to recovery

Update from me post my doppler and follow up with consultant:

Doppler: I had my doppler two weeks ago and to no surprise the caverject didn't produce a satisfactory erection, however I was told it showed strong blood flow into the penis but a large outflow; venous leak.

Consultation: I went for my appointment with the consultant today and to my annoyance saw a junior registrar and not Ralph or Dr Muneer. I had to go back over all the case history once again and how I had tried TRT, viagra, cialis, caverject all with no success. We discussed the implant after which he told me, given my age he would have to speak with the senior consultant (Dr Muneer). He came back into the room and said given my age (27), the implant really is a last resort and all other options need to be exhausted before this. He said he is going to put me on a waiting list for venous ligation surgery to which I told him I had read the success rates were poor and patient satisfaction low.

I am feeling pretty down but would appreciate any comments surrounding venous ligation surgery. I am yet to see any posts of it being
successful and a few which claim it resulted in a shortened penis due to scaring. Any advice from my FrankTalk brothers?

starofpramen I am not a doctor so I could be in a wrong ( Although it must also be said that many urologists also have no idea) but reading a lot of medical literature, the problem with ligature for venous leak is that if your leak is congenital (in example since all your life) the surgery It is more likely to fail because there is tiny veins which can not be tied and they will grow "sick" if your escape is congenital but if your leak is due to a injury or something similar and if the doctor tie correctly the vein or veins with leaks (all of them, the problem is to know the veins which are bad) you will have many more possibilities to be well or better in the medium or long term, because I understand the new tiny veins or the other veins not tied they will not have affected its closing valve and they should work well.

Then I think this surgery is very compleoux because if your venous leak is congenital the chances of a cure are low, and if your leak is from a injury your chances of cure are higher but the problem here would be to find all the vein with malfunction, because if the doctor leaves a diseased vein without tying, the blood will escape through that vein, although that vein will be very small in the long run it will become bigger and all the improvement will go away
Because of this I think doctors offer this surgery to the youngest, because many of them think it is because of an injury, but if they dont tie all the bad veins I think it will not work I guess

wish you all the best Star. I had my implant at 50 after suffering the effects of ED for the last 5 years (really more like 10). I can tell you that given the way I feel now, I would have gotten the implant at 15, cos it basically resolves all of the issues that pills, injections and pumps cause. But as the brotherhood have said, get a great not good but great surgeon. A high volume guy would have seen all possible issues and can deal with them as they arise. When I first considered getting implanted, I consulted with my local uro who confessed to my wife and I that he does probably one (1) surgery a year (we bolted from his office) and then I discovered Kramer, whose love for his craft and his bedside manner have no par.

Listen, you live once, do for yourself what will give you the greatest satisfaction. Having suffered from ED i know how debilitating mentally it can be.

My remaining concern is, as you mentioned, having recurrent future surgeries. Ya the recovery each time will suck, and so will paying out of pocket since my field doesn't really over nice insurance. And at our age, we cannot get Medicare, which is kinda of funny. I talked to Kramer over the phone and discussed costs of inevitable corrective surgery. Its really quite affordable, especially with a little financial planning.

Anyways, I hope this post also shows you aren't alone. I will keep you up-to-date on what I do. Most likely I will schedule surgery by September 2017 or January 2018, depending on work.

I believe that injections caused scarring, and hence caused a 35% VL according to a Doppler test. From what I have read, there is no cure for a VL, so I decided to go for implant and am scheduled for one on 6/8. if you have a VL, I'd say, go for an implant. Why waste time.

I just got it confirmed by Eid that I have scar tissue in my tunica. Or at least that's the theory. So most probably this 26 year old will go for the implant as well. Just hoping it can regain my girth...what does franktalk think? Tunica fibrosis should be worse than corporal fibrosis I think.
