Hi guys,
I recently had a penile Doppler ultrasound with intracavernosal injection and I’d like your thoughts.
Key points from the report:
Penile anatomy normal (no plaques, no fibrosis).
Erection achieved after injection, but only moderate rigidity (~7 min).
Arterial inflow looks good:
Cavernosal artery dilation ~85% (normal ≥75%).
PSV peaked at 65–66 cm/s bilaterally.
Venous findings:
Moderate venous reflux during Valsalva in the dorsal penile veins.
Official conclusion:
Mild to moderate venous leak – clinical correlation recommended.
From what I understand, blood is getting in fine, but it may not be staying in well enough, which matches my symptoms.
For those of you on FrankTalk who have mild or moderate venous leak:
What treatments helped you most?
Did PDE5 inhibitors still work?
Any success with injections, vacuum devices, or lifestyle changes?
Appreciate any feedback or shared experiences.

Ed one year.use ofpills.. doctor saidis is psychological problem.i can't get injections..MPP is the most near solution for me.Greece

Hey Jimakos - I also was diagnosed with venous leak. I don’t remember my exact numbers but the bottom line was that it’s impacting my erections fairly significantly.

Viagra and Cialis was hit or miss for me - definitely helped for additional inflow but this was never the problem for me. They did help me “maintain” more compared to my natural erections without PDE5’s on account of the extra inflow. But for me, I’ve never once achieved an erection I felt like I could “drive into a wall” as some of the forum have described :D

I tried shots this past September and October - honestly, they weren’t for me. I tried them multiple times and the results for me strangely enough seemed worse than Viagra. For me, I did not respond well to the Trimix during my Doppler. They indicated to me that this was probably because I was in a clinical setting and not turned on - but later when I tried them with a partner they didn’t make a difference either. However, for many many guys they report good success with shots. If you responded to the injection during your Doppler, you should consider trying Trimix out (if the PDE5’s don’t do anything for you).
ED since high school. Pills always hit or miss, mostly ineffective. Finally diagnosed with venous leak in early September 2025.
Surgery scheduled for Rigi10 MPP w/ Dr Hakky on January 20, 2026.

OH another thing to seriously consider trying though - a cock ring. In the past, this has helped me a bit from time to time to help maintain my erection a bit better, even with venous leak. The results aren’t miraculous or anything, but it has helped. I’m getting a malleable penile implant in January and I feel like I may continue using a cock ring with the malleable, as some forum members here with malleables have reported that cock rings actually help still with engorgement even post-implant.
ED since high school. Pills always hit or miss, mostly ineffective. Finally diagnosed with venous leak in early September 2025.
Surgery scheduled for Rigi10 MPP w/ Dr Hakky on January 20, 2026.

A silicone ring helps tremendously but there are still down-sides. It can get annoying after 30 minutes or so but at least with the implant you don't need as much constriction as when you are trying to maintain an injected balloon-cock. Having reliable rigidity to fall back on makes it easier to deal with for sex. In fact the best approach is usually to start without it while she is warming up anyway, and then just pop it on when you want to finish.
Oct 7, 2025 - Coloplast Titan XL - Touch pump - 26cm - 0.5cm RTE with Jonathan Clavell. Still sorting out TRT; plan to try a "modified natty" approach without injections.
