How many have bloodflow issues after surgery?

As the title states, how many of you guys have issues with reduced blood flow after surgery to areas like the head? I already have a pelvic floor issue due to pssd and antidepressant usage. It seems like I get less blood flow there even with cialis and it does not help it is hard for me to get aroused. All of these issues along with ED are why I am considering the implant in the first place. I have been seeing more often posts about an implant erection “doing the job” but not looking natural or naturally plump due to people having issues with still getting blood flow. Can anyone give more specifics about how common this is? I am getting extremely close to going ahead with the surgery but things like this give me major concern since obviously the surgery is permanent. I can deal with recovery and having to stretch back out over months back to normal size but permanent lack of blood flow giving a natural looking erection leads me to being hesitant, especially being I already have shrinkage issues a lot and bloodflow with pelvic floor. I along with most I have read about the last many years have had a little less engorgement initially after surgery as there’s always a little to a lot of swelling which can cause a little less flow. Generally the most of swelling is gone by 4-6 weeks but generally you won’t be using it before that time frame either. Seems that within a few months after implant I was getting as much engorgement as ever I had before. I use cialis 5mg daily or occasionally 10mg on sex days. It definitely helps get that stimulated feeling and a lot of plump up.
59 years old ED started mid 40s pills failed after 10 years. Injections works but diminishing results with pain. Implanted 5-22 Baylor,Scott,and White Dallas.Dr Michael Wierschem, infrapubic Coloplast with Classic pump 20cm and 1cm RTE. Going strong

From my readings men seem to very happy how their implant looks and feels. I am in that list as well, the only time mine looks alien is right after a good deflate, but within the time till my next piss he looks good.
Sitting in a relaxed state he will have that feeling of comfort and arousal to the point I text my wife and flirt with her.
You will definitely need to work on your pelvic floor after implant.

I’m only 11 days post-op. My experience right after surgery was that my head looked shriveled, sunken, and bloodless. I was worried about that and wondering if that was my new normal. But in the last 4-5 days my head has puffed back up, and my shaft looks a little bigger too. I think initially surgery makes the tissues lose some perfusion and fullness, but over time it seems that things return pretty quickly to whatever “normal” is for you. At least that is my experience so far.

Thanks everyone for the replies. Definitely one of my largest concerns. I’ve tried to do physical therapy for my pelvic floor but nothing helps. It’s almost frozen in a semi contracted state. I think it is a permanent nervous system issue after stopping antidepressants quickly. I have the same issue with my bladder sphincter muscle as around the same time this started I had a weak urine stream. I went to the urologist and he did a cystoscopy and said my bladder muscle does not relax like it is supposed to, so I’m thinking it’s all related. It’s been like that six years now. It no doubt affects me getting an erection and even makes my sensation very low. I’m to the point of giving up on it and just going implant. Heavy doses of pills can work occasionally but most of the time I use injections which of course are not fun. I’m just worn down and tired of fooling with it. I just want to make sure I don’t make a quick decision being that it’s permanent and regret it or my wife ends up regretting it. I’m 42 so I would have to live with it quite a while lol.

Mark, No one can be certain what your individual outcome will be, but I think most of us are happy with the way our penises look and perform with an implant. Once reasonably well recovered from the surgery, and into cycling, the implant does much of the heavy lifting with engorgement as it inflates the corpora artificially. The glans usually remains soft and doesn't engorge unless stimulated by arousal (or using things like Cialis, Viagra etc which you have said doesn't work for you). If the implant cylinder tips are far enough into the glans it can help improve firmness there; at least it has for me. I also find that being very aroused causes some overall engorgement, including the glans, especially just before and during orgasm. I think both my erect and flaccid states look natural. I can't really tell the difference from pre implant other than being larger flaccid. p.s. Have you looked into Botox injections for your bladder issue? Long story but I had minor but persistent continence issues after my radical prostatectomy. It was considered to be overactive bladder, and I think probably also some sphincter nerve damage and pelvic floor weakness. Pills didn't help and I had already done Kegels to death. In the end having Botox injected into the bladder wall via cystoscopy to relax it got me to 99.9% dry. I'm assuming it could work for a weak stream as well.

Finally implanted Mar 6 2025 AMS 700 LGX 21cm x 12mm, no RTEs, MS pump, Penoscrotal.
Recovery great so far but have a bend.

Thanks I’ll have to look into the Botox. I have done Botox for Ed but it didn’t help. I don’t know that I could go through with a cystoscopy again unless I was put under. It was brutal.
I have a hard time getting aroused do to my pssd. That’s probably my largest issue so bloodflow to the glands worries me although I guess if enough cialis helps me get an erection currently it would be enough to send blood there after surgery?

There are options. 5mg, 10mg, or even 20mg of Cialis every couple of days should help plump you up. For arousal you can try drugs like Cabergoline or supplements like Yohimbine.

You said you were thinking of getting an implant “so you don’t have to fool with it anymore.” Just a caution: the implant is a permanent solution, but it’s not set it and forget it. You have to be vigilant for complications, infection, device failure, and other issues. At your age you’ll likely need 3 or more surgeries over your lifetime even if everything works well and you get good device life.

Implants are wonderful, but not perfect. It’s another step on the road, but not a magic wand.
