I am still hesitating. I am currently on 5-10mg of Cialis per day. Gives me really good erections at night, but spontaneous erections from sexual erection have become almost impossible, it always requires prior manual work.
Last weekend I had an experience that made me rethink my whole situation: I wanted to perform properly and took 20mg Cialis - not much difference, when it came down to it, I failed.
Now I'm thinking of skipping the injections and switching straight to the implant... my erective funktion is too less to live a good life, but, on the other hand, too much to give up... Silly situation.

I would use everything I could before getting the implant, yes the implant works and I have no regrets, its been fantastic, but thats from coming from something that quit working naturally and with pills and with shots and with uretha inserts.. I wouldnt do the uretha inserts again, if I had a working pecker. Anyway, use what you got as long as you can, you know your length, your girth, your glans still working when aroused. The day will most likely come when you get the implant but I would use what I had as long as I could.

Like you I thought that getting an implant would mean never having an erection again. That it would just become a mechanical thing of pumping up.
I got an implant 6 weeks ago. I STILL get erections around the cylinders. If I see something that turns me on I get engorged. I still feel the stirring. I still get morning wood. I feel like I have the best of both worlds.
While you are researching surgeons and having consultations try shots. If they work perfectly for you, maybe you’ll decide to do that for a while. At least you’ll have the experience. And it won’t really delay getting an implant if you choose to go ahead.

I agree with GoodWood. At least give injections a few tries. I find them very inexpensive, convenient, good for spontaneity, 95% reliable and gives me a rock solid erection. Downsides are risk of priapism (but phenylephrine is a sure antidote at home) and risk of fibrosis (variable results for different men and not much agreement on risk). The actual injection process seems intimidating but, In practice, it's a non-event.
Born 1954. Diabetes, hypertension and atherosclerosis. Sildenafil is iffy. Tri-Mix (30/3/20 Pap/Phen/PGE1) a godsend pending long-term efficacy. Daily Cialis. Tried LiESWT, Botox, PT-141, Eroxon, QST, DUS, Vertica, cabergoline, psychotherapy+hypnotherapy.

I'm the same as you, medications sometimes work great but sometimes they don't. Sometimes i can be really hard and everything is going great, then all of a sudden, bamm, totally gone. I'm personally sick of the mental gymnastics that come with the pills. I've decided to completely skip injections after the feedback that i received and from my own research. Injections increase the chances of PD by causing scarring and fibrosis and in turn can make getting an implant more tricky. So many guys on here say they wish they had just gotten the implant 10 years before they did it.
I have my first consult with a high volume implanter here in Australia in July!

43, ED since late 20's, Pills on and off since then increasing strengths with inconsistent results. Now 5mg Cialis daily and either 20mg Cialis or 100mg Viagra
Not sure if it's Psychological or Physiological, long-term use of SSRI's probably hasn't helped

If it's not enough for action, it's not too much for an implant, IMO.

That's the part that I find the hardest, it's such an insidious thing ED. It plays with your mind so much, one time things work great, then the next time absolutely horrible. You find yourself asking yourself all these questions, what did I eat that day?, did I have too much caffeine?, did I sleep enough? It makes it so hard to make the call on the implant, I can definitely relate to where OP is at but now I've decided I'm definitely doing it now

Sometimes I wish I could slip below the threshold that would give me a reasonably good erection under certain circumstances - then the decision to get an implant would be easier

What's the nature of the nerve damage?
Nerves can regrow sometimes but if it hasn't healed in 10 years it seems unlikely that it will.
I wish I could go back to when I started injections. My dick still worked great with pills much of the time, it was more a mindset problem. (I've been single for many years so every time I had sex was a risk of situational ED.) But once I started injections they were such a sure thing that I overused them and ruined my dick to where even injections didn't really work. I got only two decent years out of them before things very quickly deteriorated. I should never have started them.
