67 yo Gay. Used Viagra from the 1990s till 2019, and then it was no longer effective.
Jan 24, 2025 consulted Dr. Eli Thompson of Portland, Oregon. After discussing options, I agreed to get IPP. This is a diary of the journey.
Mar 25, Day -1: Sterilized the house, laundered bedsheets and garments, to prepare for an infection free recovery.
Mar 26, Day of surgery: arrived at Good Samaritan Hospital at 530am for a 730am procedure. All went as planned and surgery was over by 9am. 2 hours in the recovery room, 90 min ride home with a friend, and settled in. Just soreness in the scrotum area.
Took some Tylenol and applied ice pack, covered in dry towel for several 20 min periods. The penis is partially inflated and gauze bandaged. Dr. pointed it north and placed a jockstrap to hold in place. No catheter, able to pee with a little burning sensation, the pee does scatter around the toilet from the odd fixed angle. There is a drain pipe and bag for bleeding. Emptied it at hour 5. Have to keep the bandage and drain pipe in place for 48 hours.
Mar 27, Day 1: Slept okay 9pm - 7am, but got up 3-4 times to pee. Urination burns a little, probably from the catheter used during surgery. Drink water frequently to keep hydrated and urinary tract flushed. Applied ice pack for 15 min, with dry towel, each time up to pee. Pee falls all over the toilet area b/c aim is difficult with the penis pointing up. Sanitizing bathroom and bedding often.
Emptied the wound blood drain bag several times. Less and less blood is draining, so hopefully the wound is healing. Only taking 1,000 mg Tylenol (acetaminophen) for pain relief every few hours. Spending morning at home in a bathrobe. The bandage and gauze-packed penis/scrotum area feels like a football, and I waddle like a penguin! Friends invited me out to lunch, but declined with a polite lie about "work"!
Day 2: Urine no longer burning. Using a plastic urinal bottle to avoid spraying all over. Removed blood drain tube and bandages from incision and penis, which is standing at 90 degrees! Felt good to take a shower. Had first BM since surgery, and that seemed to reduce pressure down there. Ordered 6 jockstraps since penis must be kept pointing up for 5 weeks until Dr. activates the device.
Dr. Thompson texted to say that surgery went well. He installed a Coloplast Titan 20cm, with a 1cm RTE.
I'm applying ice packs wrapped in a towel every hour or two, for 20 minutes. The swelling and pain is gradually receding. Only taking Tylenol, and spacing that longer and longer. Was able to drive to a store 10 miles away, moving slowly and holding onto the cart for support. Needed sock puller to put on socks. Skechers shoes are easy to put on without bending over. Since the crotch area is tender, bending at waist is difficult, so using leg muscles to get up and down to pick up things. Laundering all clothing frequently and using Lysol wipes/spray in all living areas.
Day 3: Pain and soreness has abated more. There is some bluing of the skin in the pubic area. Decided to cut back on icing - I probably overdid that! Taking much less Tylenol now. I find that lying down is better than sitting, in terms of pain. So, I spent more time in bed reading. As scrotal swelling starts to go down - balls still the size of a medium grapefruit - the penis is starting to point down a little. Gravity also helps drain swelling when in bed. Junior is standing at an 80 degree angle!
Tried on some pairs of jogging shorts, and they felt comfortable. I don't want to be in a bathrobe all day in case of visitors. When getting up from a bed or chair, I put weight on the arm and leg muscles, favoring the abdominal/core muscles close to the groin. That prevents shooting pain from the swollen organs.
Day 4: I learned that laying flat on my back with a couple of cushions pushed under the butt raises the swollen scrotum above the heart. Gravity works and the fluid goes back into circulation, reducing swelling. Spent most of the day on long planned driving, and sitting in car/restaurant/theatre activities. Unable to ice much, so depended on Tylenol andIbuprofen (NSAID painkiller) to get through the day. Swelling was high at the end of the day.
Day 5: Swelling is lower, balls the size of a large orange. The blue discoloration in the pubic area just above the surgical incision is slightly improved. Touched and felt the pump between the balls gently, being careful not to activate anything. Working at my desk, but breaking every hour or so to raise the scrotum and/or ice.
Day 9: Swelling and pain has receded every day. Started taking two warm baths a day, and pulling the pump down gently while in the bath. Tried using jockey shorts for a couple of days, but decided that jockstraps hold up the scrotum and penis more snugly. When I move around during the day, or turn in bed, having the "junk" not jiggle around reduces pain. I do place a small sock b/w the penis and the jockstrap for softness. I am now able to do most daily activities like driving, shopping, working on the computer, and visiting friends. Not ready to resume gym yet. Get up about 3 times/night to pee, and apply an ice pack. Also lay down every 3-4 hours to elevate scrotum and ice.
Day 13: Swelling and pain has receded further every day. Rarely taking Tylenol. Still taking two warm baths a day, and pulling the pump down gently while in the bath. Icing and elevating a few times a day. Doing more daily activities as the scrotal area complains less and less. Starting to feel around the pump to get familiar with its shape, but being careful not to hurt anything in the still healing tissue. Also gently moving penis up, down, sideways, and in circles a few times a day so that the cylinders settle in with the surrounding body tissue. The only remaining visible swelling is in the foreskin area. Although I am circumcised, there is a little bit of foreskin that has some fluid. The scrotum also swells a little after sitting for a time. I lay down and elevate/ice to reduce that.
Day 30: Swelling mostly gone, and just a trace of tenderness in the scrotum around the area of the pump and tubing. Back to most daily activities as normal, including gym - treadmill and some upper body strength. Will see Dr. Thompson for activation in a week, at the 5 week mark. Able to ejaculate normally - have done so 4-5 times in the past two weeks.
