I'm no insurance expert, and there's no one-size-fits-all with all the plan tiers, but from what I've seen, Aetna and Cigna can be a pain in the butt, when it comes to covering penile implants. On the other hand, BCBS tends to be much more accommodating.

Depending on the plan BCBS Texas can also be terrible.
Bottom line: before you sign up for any insurance you need to ask and be told that CPT 54405 is covered.

With Aetna, the number of denials increases significantly a few days before the procedure, even after receiving confirmation and pre-authorization for penile prosthesis procedures (CPT codes 54400-54417).
