It's been a few years since I've posted here, but I don't know where else to go.

Following a prostatectomy in 2016, I couldn't attain an erection. I tried Cialis and other meds, attempted the shots (I just couldn't do it) and subsequently developed Peyronies in which my penis bent upwards into my abdomen. My surgeon suggested a dual procedure in which they'd fix the Petronie's and install an implant at the same time. The implant has worked successfully but because of the Peyronies procedure, I lost at 2.5 inches.

Recently, I've read that an implant itself can correct Peyronies disease. Did my surgeon make a mistake here? Because of the shortening of my length, sex has been nearly impossible for my wife and me (she can't be on top due to knee injuries). For a few years now I've nothing but regret in having this implant, as it seems like a wasted procedure.

I have a follow up appointment early this next week and wanted some counsel from you all before I speak with the doc.

Howdy Professor. You have no idea how tough it is not to ask “How’s Mary Ann?” Although that would betray age and a certain lack of good choice in the TV we watched in the mid-1960s.

Turning to your issue/problem. There are a few pieces of puzzle missing. Follow Paul Harvey’s lead and give us “the rest of the story.”

You told us when you had your prostate cancer. When did they put in your implant.

Truthfully,it would help if we knew who the surgeon was, what kind (make/model/size to include RTEs) of implant, when the implant was done, how big you were beforehand, and how faithful you were about cycling your device post-surgically. Was your surgery done infrapubically or penoscrotal let?

We (on the board) have heard of the occasional case of Peyronie’s where a surgeon has been unable to get enough implant in (due to fibrosis or scarring in the corpora) and a man is left undersized. This isn’t common but it has happened.

Most implant recipients report an immediate size loss (2” isn’t unheard of) but a gradual return to their original dimensions as healing occurs, swelling goes away, and they operate their implant causing tissues to stretch to very near their original size.

Question: when fully inflated, gently palpating the underside of the head, where is the end of your cylinders?

What procedure was used? My 40 degree dorsal was corrected using the procedure termed plication of the penis. Length went from @ 6 3/4 to 5 1/2. Initial install was May, 22 and revision Jun, 23. Cycle and VED daily but extremely slow process. I do not have a large fat pad so I manage to get good use of all 5 1/2 of my perky guy. PLEASE note, due to the nature of plication there will always be a loss of length. Rgds

Cycle your implant aggressively. Most ppl gain size when it’s time for a revision.

I had Peyronies curvatures of about 30 degrees up and left prior to my implant. Immediately after surgery those curves were gone but I'm about .5cm shorter on the left than on the right due to Peyronies, which is not noticeable. I did not have plication performed. I believe Dr. Eid has a photo of a patient who had a 90 degree Peyronies curve that he was able to straighten out over the course of a year with the implant.

Be sure to consult with a top surgeon like the ones mentioned in this forum to get the right answer. From what I read once you have plication then you definitely lose size that you can't regain.

I had my prostate removed in 2016 and had the implant/peyronies surgery in 2018. I had about an 90% curvature. Sex was painful for both of us when I was on viagra or Cialis. I was about 7.5" before surgery and now I'm maybe at 5". Following the insertion of the AMS implant, I did cycling like crazy, with about 15-20 pumps each time. I did notice with that the glans remained a bit soft after pumping (and still does). Because of my insurance, I had to use a local surgeon rather than going to the big guns like so many of you. In 2022, I had to have the pump replaced due to a malfunction and it's working fine now.

My biggest concern is that I had both procedures done rather than just the implant. My surgeon told me that due to the severity of my case, the peyronies surgery would cause some distinct shortening and the implant couldn't help extend my length.

I've basically given up on sex because it's just too frustrating. My wife of 35 years has been wonderful through this whole thing.

5" is about the world's average penis length I believe... If you've really got 5" still it should definitely be usable for sex. Is one or both of you extremely overweight?

I would not necessarily classify his decision to plicate as a mistake. Surgeons who only perform a small number of implants do not have the experience and knowledge of the high-volume experts. Quite often the local urologist only uses AMS. When I had a video conference with my surgeon, I had already narrowed the choice down to the AMS CX and Titan, both of which he uses in his practice. We decided that the Titan was the unit of choice given my Peyronies. I would not have know of either one of these had it not been for FT. I signed a consent to allow my surgeon to perform plication if he deemed it in my best interest once he inflated me in the operating room, but he was confident that the Titan would resolve the issue (with a small loss of size on one side).

I would say that most local surgeons would consider plication as the gold standard of resolving Peyronies. It also depends on the severity of the curve. We really don't know if one of the super doctors would have decided upon the same treatment.

Because of the wife's bad knee we bought a sex swing. Helps limit pain in my bad back too. The swing has been great to enjoy each other without fighting the bed.

My wife has gained some weight over the years, and with her knee problems she can't be on top. I just measured and I'm barely 4". I guess the report of 5" was wishful thinking. LOL
