tight scrotum \ pump placement and a few other questions

Postby Islanderboy58 » Fri Jan 12, 2024 4:12 pm

Wed the 17th I will have my consultation with Dr Levine in Chicago I'm feeling good about him as my surgeon, however I am concerned about pump placement with whatever implant he uses. I have always had a tight scrotum ,rarely hangs loose so my concern is how hard it will be to get a grip on the pump and squeeze it .
Also I hear so many comments about size loss and gaining it back during recovery ...it sounds like I should I expect it to take a year or better before I get back close to my pre op size? I am very average 5 3/4 length about 5 girth I lost a bit over and inch length( formally 6 3/4) due to ed over the last 5 years and hope I might regain some of it with consistent cycling after surgery.
Also hoping maybe to go with Titan if he says its a good choice for me.
Thoughts?
Many years on Viagra until it didnt help,I moved onto Trimix 5 years ago and it now has limited effect.Time to move on to The Implant.

the pump is larger than my each of my nuts. It will probably fit but he will tell you more. The Titan is good for girth for sure. Are you using a VED prior to surgery? I have loved all my Titans. I am in failure mode right now but hoping for another Titan on Feb 27 surgery date!

I lost about 3/4 to ED as well, the implant so far, 7 months post op, gained back 1/4 to 3/8ths. I have a “small” scrotum Clavell said, pump placement is nice, below the nuts closer to the taint. But, your right, if i am cold and that skin is tight its harder to pump and deflate. Its not that much harder though, nothing to worry about in my opinion.

If you want that length back people here on ft like vacuum pumps daily before surgery, maybe ask your doc. For me i think hanging weights like a “bib hanger” or “jes extender” those types of devices, there are many, work better i think.

Yes pumping as much as I can its new to me , I lose the erection as soon as I release vacuum though.

What is the nature of the failure?

Not sure what the failure is from. The doctor thinks it may be the tubing but I think its the left cylinder. All the fluid is gone :(

I have the tightest scrotum ever. One thing that I wish I had done, and will do when the time comes for a revision, is request that the pump be placed so that the release valve is set to squeeze side to side instead of front to back. Dr. Hakky said that he could have placed it like that had I requested before the surgerybut I had no idea it would have been easier to squeeze like that. When you have a super tight scrotum, it can be more difficult to reach around the back of the release valve to be able to squeeze it. If the valve were side to side (left to right) it would be much easier even when my scrotum is really tight. That’s just my opinion for my own physiology.

like Nebraska said, you can talk to your surgeon about placement.
During recovery, it's good practice to gently pull the pump down (as if you were standing up and pulling it toward the floor)
and also pull it forward, gently of course.
This helps ensure it doesn't get bound up too high to pump and deflate.

My sack can go pretty loose and even if it's cold/tight, I have always found a way to sort of rub my finger up around behind my nuts and grip enough of the pump to pull it down away from the taint to pump it up.
Finding, tugging and pumping... is truly a fine art lol. Just takes practice :D

Islander, I thought I had a small scrotum also, but mine fits fine. I have the Boston Scientific AMS LGX 18cm with 3cm rear tip extender.
Depending on your internal measurements, proximal (inside your body) and distal (outside your body) when the doc does the install will determine if rear tips extenders are needed to make sure the tubes are long enough from the bottom of the two implants so the pump can be placed as far down in the scrotum as possible.
I'm not single so I wasn't concerned about the pump being hid at the back of my scrotum, mine sits between the two boys in the middle, so I can easily access the pump and hold it with my left hand while I pump with my right hand.
I'm a road bike cyclist, so I was concerned about the pump causing pain down there between the two boys, with rubbing or pinching, but I don't have any of those problems.
As far as size loss.
I had total ED for 4 months after the prostrate procedure that I had until I could get the implant installed. I used a VED every day to stretch my dick and keep the blood flow going. I had 6 1/4 inches with the VED, initially after the surgery I was at 5 to 5.5 inches when I first started cycling. I gained back my pre-op size back around the 6 month point. It is very important to do cycling to the max inflation when you are given the go ahead to cycle. There is a surgical capsule that starts to form around the implant and you want to make sure that you stretch your dick tissues as much as possible before the surgical capsule has permanently formed, this takes from what the doc said to me 5 to 6 months. It's also important to deflate all the way each day to expand the reservoir because a capsule is forming around that too, and if you don't deflate all the way the capsule will be too small for the reservoir to fill completely.
I had my implant done at a military hospital and my urologist used Boston Scientific products. I was supposed to get the CX, which is stiffer, harder and has more girth, but they decided in surgery to give me an LGX because of the measurements and they wanted to allow me to gain as much back as I could for length.
I think there was another military urologist on staff that used Coloplast products but I wasn't given that choice.
I think both products are good, so do some more research and see which one you want to go with or trust your doctor with his choice.
I'm just guessing but from your Frank Talk user name, you were born in 58 like I am, just a guess, I could be wrong.
I'm at 2 years and 2 months out from surgery and I love my Bionic dick.

Just some observations. While the Titan cylinders are larger when new. The material does not stretch very much. AMS advertises that the LGX stretches up to 25%. They don't advertise that the CX also stretches in girth & length. But it does but not 25%. Myself & several others have 6" girth with CX's. The key is that if you had decent girth before you'll have it after. Basically, if your tunica will allow girth you'll have it after surgery. My dr did tell me how much it can stretch but I don't remember the number.

My AMS pump is placed so the SINGLE deflate button is pointing outward. It only needs a single push to deflate. I do not need to hold it untill it is completely deflated. It has never accidently deflated.

My point is that neither implant will turn your average size dick into a porn star dick. It sounds to me like lots of men are hoping it will. I firmly believe that all the currently avaiable implant models sold in the USA are good. Each has its good & not so good points. Just please be honest in your expectations.

Just a side note. Titans are almost unobtainable in my state. The leading drs all use mainly AMS products. There is one that will use a Titan on request. Mine wouldn't, I didn't want one anyways so it worked out good for me.
