The size obsession is crazy
I was born in 1974. I've had venous leak ED since early 30's, but managed with pde5 inhibitors until mid 40's. I have fairly severe hour-glassing, but no hard plaques. My urologist is worldwide acknowledged expert Dr. Laurence Levine

Yeah, agreed scrotal approach is the optimal approach for maximal length according to studies.

"Compared with infrapubic, transcrotal placement resulted in a longer total prosthesis (22.3 cm vs 20.6 cm, P < 0.0001)..."

That's a different of ~2.5cm, so almost an inch.

One reason Perito likes the infrapubic because it's so fast. This to me says it's not necessarily the best option for the patient, but it's more efficient for the doctor and surgical staff.

In one of his MTP videos he clearly states he likes it because it's something like a 15 minute surgery.
2/22/23 AMS 700 CX 21cm + 1.5cm RTEs. 58 yrs old, wife of 37 yrs. Penoscrotal. 100ml Conceal reservoir. Dr. Clavell. Pills failing and went right to implant, skipped the injections. 12 mos. later: 7 1/2" x 5 3/4"

How did it become a trash talking session about how pensoscrotal implants are "better" than Infrapublic? :roll:

Just sounds like a lot of self-reinforcing and self-serving BS to me. :(

Where exactly is the perforation, and what symptoms prompted you or your doctor to suspect the problem.

My erection is also at a fairly low angle, around 4 or 5 o-clock. I don't have any significant pain in the proximal area, and everything seems to work fine, but I'd prefer the appearance of a nice straight out erection if possible.
Age 59. AMS 700 LGX 21cm + 2cm RTE.
Peniscrotal, 2/23.
6.25" OEM with Cialis, about 5" post-op. (11/01: Length 6.75" and girth is up to 4.75" mid shaft.) Still growing, albeit slowly. :D
Cycling daily. Hoping to get to 7". ;)

Hi Dave

If the angle bothers you a lot, there is always the option of getting a suspensory ligament tightening (stitches above your penis). It will give you the angle you wish. I just had this done.
If your angle doesn't give you any problem, i wouldn't risk it though.

Early on in penile implant surgery men were dilated with different instruments. And perforation of the Tunica was more of a problem than it is now. Weather it be a crossover or proximal or something else. Thing have improved and new instruments have been made. Rounded smooth dilators have been designed and are now in use to prevent this. Really good doctors had a feel for this and the reason they had less reoperations was partly due to this. Used to be a perforation was a game over occurrence but now there are sling an sail repairs and other ways of resolving a perforation.

When I had my cath removed the morning after surgery my penis flopped to the left flat against my body. I spent the night and the Resident had this very disturbed look on her face and she looked to the doorway and my doctor was there and had this strange look also. I had always pointed straight forward. No curvatures or angle to the left or right. After surgery I was almost 45 degrees leaning to the left at max inflation. I could also follow my cylinders down inside my body and the left cylinder was closer to the surface starting under my scrotum and back toward my rectum where I would have pain after sex. The proximal (RTE end) of the cylinder was not inside the Tunica and was not supported. There was a perforation of the Tunica that the RTE went through. A sling repair was done. A better name would be stirrup as suture material was used to make a stirrup to support the proximal end of my left cylinder. My angle is a lot better. Well over 50% improvement but still not perfectly straight. A bit a left angle from the base. And the pain is over 50% better also. Again not 100% and I still need a bit of caution or I will have pain after sex. I can now follow both cylinders back through my scrotum and they are both the same in depth under my skin. This was side to side angle and I had about a 4 o'clock slightly down angle. That was not a big deal for me. My discharge instructions said I did not need to tape my penis to my stomach pointing at my belly button. I could not have done that for my plane ride home any way. That would have only been possible for the time I was flat on my back.
Injections failed. Implanted 3-21-18 AMS 700 LGX 21 + 1 RTE 100 cc reservoir 6.5" L 5" G Dr. Kramer.

Proximal Perforation Sling Repair 4/13/21 Dr. Broghammer

66 years young.

Go for AMS. Problem with Titan it's hard to hide and looks like a semi. Could give for example problems when you are around your grandkids.

I am scheduled to get an AMS 700 on April 6, 2023. Dr. Blick in Phoenix seems to be in bed with Boston Scientific and never mentioned any other brand. Sorry I did not find this website sooner. Anyone have any experience with him? He did examine the size and shape of my penis. I am 81 and 3 yrs post radiation and androgen denial.
