After fighting ED/DE for years. I'm contemplating getting the surgery. My Dr. gave me two options last week. Increase Caverject or IPP. I choose increase cav but I hate the injections it still doesn't get but 60% hard and it's somewhat painful and stays irritated. I called back and asked for a consult with the IPP doctor. So it's been sent up. This is from the Durham VA hospital. Anyone with experience from there...who do they use for this surgery I would like to research the doctor. Viagra, Cialis which the VA prescribes 2 or 4 pills a month weren't cutting it so I like a dummy started sourcing them from the dark web and now I'm afraid of fentanyl and stopped doing that. At this point i just want to be able to perform even if I loose some size. I gave a little more info in the introduction section. You all have been a wealth of information and are truly bringing me out of a dark place mentally.

Ok. Before you jump in to the surgery line, I have a couple of questions.

You mentioned Caverject. Not only did I play Army for a living for 23 years, I’m a 31 year veteran of sticking a needle in my dick. This is an area I know a little bit about.

Have they tried Trimix or Quadmix instead of just Caverject? I did Caverject for about 5 +/- years, it worked (not well, but it had an effect, especially when paired with the brand new Viagra pills about 1998). But for me, nothing beat Trimix for many years. It was reliable, yes it had to go in the refrigerator, and it gave me decent erections for many years instead of The achy dick 75% erections I got from Caverject.

If they haven’t tried the Trimix (or Quad), then you need to see if that has any success.

Have they judiciously suggested combined shot and pill, or something like daily dose Cialis with a shot? Yeah, some guys have to watch for priapism. Most guys with severe ED don’t seem predisposed to priapism. Still, you want to do things like this only under the advice of a medical doctor in the amounts he or she prescribe.

If you’ve exhausted those options, and still don’t have satisfactory results (even if a bit inconvenient) then yeah, maybe it’s time to talk implant.

I've tried Trimix and Caverject. With the VA they kind of send you what's in stock. So it varies quarter to quarter what they send since it's a 3 month supply. Both give about 75% and both equaled pain after injection no matter the side and how far down/up the shaft. They didn't tell me to combine the pill and injection but I have been doing that for about 2years to achieve the blood rush where I can put the ring on to hold some blood in. It's to the point where I've held a death grip at the base to get it in only to go limp after a couple strokes. If it wasn't severe enough the VA won't take you down that avenue at least it's been my experience. Now I'm fighting the mental part of having to rely on the mechanics of something but I think it will get me to the more "normal" sex life than not. My wife and I are pretty active when I can get things operating.

I’m not a doc. All the usual disclaimers apply.

Based on what you’re saying, you might just be ready to go implant.

Don’t get frantic if the VA doesn’t fly you first class to New York, put you up in the Ritz Carlton, and contract you out to Dr. Eid.

There are quite a few guys on here who had a VA doc perform their surgeries very successfully. So there are people in the VA system who do this well.

There are any number of surgeons who do a good job of implanting. We all talk “high volume doctors are best” but I had an excellent outcome after surgery from a guy who has probably done 1200 in 10 years…..at least as good as by a different doctor who might do close to that many in a year.

So talk to whoever they refer you to. See if what they say makes sense to you. Ask any and all questions. See what kind of vibe you get. I don’t think the magic is in the doctor. The key is having confidence in whoever the doctor might be.

I hope that you have success and it sounds like you've exhausted all other options. As best I can tell it does work out for most people but please do keep in mind that there are risks. I had the surgery in April of 2022 and I now have chronic pain in the abdomen and prostate area. It is not getting better and it's actually worse after sexual activity for a few days. Just remember if it goes bad it can go real bad.

The injections gave me two bouts of priapism, one that was so nasty I knew after it was over, “there’s only one choice for me”. My life is indescribably better. I have my issues in life, but nothing compares to having this burden lifted. As others have said, the high volume docs have really good rates of success. For me, I had no other choice but to try. I regret not looking into it at 30 and waiting until 36.
