When there is nothing to go back to, your decision is easy. If you still have someting working that you would regret losing if the implant should fail, then the decision is hard.

It sounds like you need to do something before you lose a wife.
If you get hard watching porn, and can masturbate 3 or 4 times a day doing that, are you using all your sexual energy on that and not saving any for the wife?
An implant is forever. If you can still achieve erections, it's doubtful any doc would implant you.

You are very functional in you natural state, and you don’t need an implant.
It would be sin to destroy your god given function.
Change your antidepressants, try others, Wellbutrin has very good sexual side effects.
Just my 2 cents.
