Implant size vs actual penis size

I have read many comments here about different sizes of penile implants. They are marked as 18cm, 20cm, 26cm etc. but I guess that these numbers do not represent actual size of an implanted penis, since a part of the penile implant is positioned inside the body. What is the scale I could use to figure out which implant size would fit me the best? What is the usual ratio of part inside and outside? What are the numbers here?

It Varies, commonly it’s 1/3 in and 2/3 out.

So a 21cm would roughly equate to 14cm.

However, all of us are different.

I was 8’ pre Peyronies and 6’ pre op but only got an 18cm.

I should be around 12cm erect but I am already 14cm and I’ve not begun cycling yet.

Another way to look at it is, with a good surgeon you should end up with what you’ve got now.

The ratio most used is 1 inside vs 2 thirds outside. But it is not all that accurate. I have a 21cm LGX with 1cm RTE for 22cm overall. 22cm = 8.66141732" x .66666666 = 5.77". But I am 6.5" at max inflation with hard bone press measurement. And I have seen both longer and shorter measurements with 22cm overall length.

It varies as Coloplast are available in 2cm increments and larger sizes and AMS are in 3cm increments. They both have RTE's in .5cm increments to bridge the sizes of cylinders.

So how does one determine which size of penile implant should he go for? Is it based on my natural erect penile length? What if someone going for an implant haven't had erection for years and he doesn't know which one should he choose?

The patient doesn't normally choose the size. Sometimes the patient doesn't even know what brand of implant they are getting- At some level, that may even be considered "a dereliction of duty-to self". Often you can get a rough estimate of size in cm. (then multiply that by 2.54 to get inches) in your presurgical visit. The final measurement is actually done in surgery. I have never heard of a patient being "awakened" during surgery to ask for final approval of the size.
Me? I had a stretched flaccid penile length of 7 inches pre-op. I had been using a VED for a considerable period of time (a year?) prior to surgery. I still do the Dr. Perrito post implant exercises.
Sixteen months ago I was implanted with a Coloplast 22cm. with a ratio (50%/50%); I had never heard of that ratio and I still don't fully understand why. that was my result. I have 11cm. inside-proximal and 11cm outside-distal. I still do cycling with varying bone pressed measurements but over 7 inches.
It seems like there are many gyration of this whole experience and what to anticipate. The best way I know of is to learn as much as you can ahead of time and to choose your surgeon carefully.
I belong to a PPO and my urologist said he would refer me to as many surgeons as it took for me to feel comfortable making related decisions. I saw 6. Yes that may be a little anal.
I am very please with my choice. I even called his mentor for a reference. He is not one of the "rock stars" often mentioned in this forum but he is certainly a "rock star" to me.

Up4Real -- couple comments please: I think you multiply cm by 0.4 to get inches, though I wish it was 2.54!
Re the proximal/distal ratio, I agree that it's weird. Mine was 8 cm proximal, 11 cm distal for 19cm cylinders -- way off the 1/3, 2/3 ratio... Through lots of cycling and perito exercises I have been able to get 6+" bone pressed at 2+ weeks out which was my pre op after ED.

For those with Peyroines-induced shortening, should one expect only their post Peyroines length after implantation or is there any hope of gaining former length?
