Insurance and implants

[Whywhywhy]

Is there any insurance that doesn't require a diagnosis for a year prior to getting coverage for implantation? Have anyone gotten implant coverage without having to wait a year with a diagnosis? Are revisions covered by insurance? If I were to get my first implant without insurance coverage, would each revision later not be covered by insurance?
