Keen for some input here as I've never seen this discussed.
My dick was ALWAYS 6.5 inches at skin pressed, not bone pressed. I then measured myself at bang on 6 inches a few months ago. Figured I had the dreaded ED shrinkage, so I bought a vacuum pump.
However, I then wondered that cos I had put on about 28 pounds recently (during lockdowns etc.) I wondered if the weight was causing the length difference. I am skinny and the weight gain was very out of character.
As a test, I lost 14 pounds and low and behold, my penis was measuring 6.25!! I am now on a mission to lose another 14 to get me back to 6.5. (Or maybe lose more as I am thinking with a fully flat stomach my penis could even be bigger than 6.5?).
See in terms of getting fitted for an implant, does any of this even matter?
The test is gonna be bone pressed, but surely the amount of fat there (even a small amount) is gonna mess up the measurement?

Implant size is determined by the sum total of the distal measurement and proximal measurements taken by the surgeon when your on the table.
Loosing weight can certainly improve the outward appearance and possibly yield a better measurement from fat pad to penis tip. So loosing extra weight is beneficial for many reasons in addition to a better outward appearance of your junk.
But your total body weight doesn’t alter the measurement from crux to where the implant tips seat in the glans which is how your implant size will be determined.
Loosing weight can certainly improve the outward appearance and possibly yield a better measurement from fat pad to penis tip. So loosing extra weight is beneficial for many reasons in addition to a better outward appearance of your junk.
But your total body weight doesn’t alter the measurement from crux to where the implant tips seat in the glans which is how your implant size will be determined.

Your anatomy, both internal and external, will dictate the length of the implant. This can sometimes be improved on with VED stretching and/or traction months before surgery. Body weight doesn't seem to have much to do with it, however, losing any pubic fat pad you have will make it LOOK longer. The health benefits are even more important.

Quincy, I'm going to read between the lines on your comment. A better health profile usually means a longer life. Thus I think its logical to presume more sex acts in that longer life. That should be a good selling point for knocking a few pounds off of the old body. Might even lead to needing a lower dosage of any meds that you might take.

I'm pasting my response from another thread were I ended up talking about the same issue and Eids approach to fitting an implant:
From reading on here this was how I understood how he operates:
Step 1. Pre-op length taken
Step 2. During surgery he chooses an implant size
Step 3. During surgery he then pumps you up and measures to make sure the pre-op length holds
So I was thinking that that pre-op size better be correct!
But I get it now that at Step 2 when choosing the cylinders he is actually taking another internal - more important - measurement that really determines which cylinder to use. If the cylinder size that he chooses is correct, it should inflate your dick to the pre-op size (a size that is variable and based on your current body fat).
I mean, based on my thinking, if you lost a load of weight after your operation you would fuck up your implant! Lol.
I would say if someone was quite overweight, he would take the pre-op measurement, and then simply factor in the pubic fat to determine his guesstimate.

Honestly I thought you needed to be very overweight for your penis to shrink, but from my experience it looks like its 1/4 inch for every 14 pounds of extra weight! I am stunned. Truly.
Keeping the weight off has many other benefits, you are both correct. I'm gonna get this other 14 pounds off and never allow myself to gain weight ever again!

The measurements the surgeon takes are taken INTERNALLY. Your fat will have no effect.
Your weight gain alarmed me, though. (Congratulations on being able to take it off.) I recommend you get yourself to your GP or an endocrinologist and get tested for pre-diabetes.

jump-ship, I think Step 1 is simply to give you an idea of what you'll end up with.

18cm AMS 700 CX, 3.5cm RTE 100cc res
Gleason 6 prostate cancer. Monitoring it for now.
Update: On my last biopsies the cancer wasn't found.

Fat pad only has a bearing on how you look or when measuring at bone pressed.
Fat does not compress as much as skin so it can make a difference on a bone press measurement with a ruler. .
Size of your dick would not be affected. It just looks smaller with a bunch of it hidden by a layer of fat. Why a lot of porn stars are fit and shave or groom. Makes it look bigger.
The size of your cylinders is determined by that measuring rod inside your dick during surgery. Barring no ed or other issues, you should end up the same length as before surgery.

18 cm plus 1 rte titan installed March 2019. Revision March 2020 by Dr. Andrew Todd, Richmond KY. He replaced the titan with an AMS 700 LGX 18 cm cylinder plus 2 rte for 20 cm total length.

Yeah, I get it now that there is a measurement during surgery. I thought most surgeons just did a simple "stretch test" to just pull the penis out and use a ruler. Because being undersized is so common I assumed it was down this rudimentary measuring technique. I therefore assumed that Eid's pre-op injection approach was superior.
But I am now thinking that these surgeons (that undersize people) prob get a decent measurement during the operation, but just go ahead and use a smaller implant anyway to avoid ANY possibility of oversizing.
I wish I could watch a video of an operation or even look at pictures. But I am desperately squeamish, so can't go near them!
