If your company approves the procedure then the insurance should cover it.

You need to read your policy. BCBSKS is who I have and has an online site where I can go to see all my usage and even had my tax form for having qualifying health insurance. And it has my policy in PDF form I can down load and read. You might check and see if you have this on line or request a copy of your policy if you did not get one.

My revisions were through Humana and then Medicare. I told my doctor 'if I need to be in pain to get the revision, then I'm in great pain'. He said 'not necessary'
