At what length intercourse becomes impractical?

Hello everyone! I am a young man from India. I have been suffering from severe ED for the last two years. I used to have a 5.7” long penis, but my impotence has shrunken it to 4.7” bone pressed. Nonbone pressed, I am 4” long-only. I know that I have to go for a penile implant if I want to have my sex life back. One of the possibilities after the penile implant is that I would further lose around half an inch of erect length. And if that happens then, the intercourse would be impractical, in my opinion. I would like to know the views of you people on this. Shall I go for the sliding technique for the penile implant? The sliding technique can give me an additional 3 cm of length. Complications of the sliding technique are very dangerous for diabetic men. However, I am a nondiabetic. I don't know if the sliding technique is as dangerous as for the non-diabetic men as well. Please help me out.

Is there a way to consult Dr. David Ralph via video conferencing? Dr. Ralph knows a lot about the sliding technique.

If you get a good surgeon who sizes you properly, then the implant can actually increase your length over time, especially so on your second implant. I would not mess with the sliding technique unless your size was like 3" or less.

Are there enough evidence that the implant revision increases erect length?

Yes, please see this study which answers your question: https://pubmed.ncbi.nlm.nih.gov/28537700/

Like many things on FT. Some gain, some stay the same. I personally can't tell what decides which happens. Wish I could give you better info.
Also, the 3cm from sliding is far from certain. That seems really risky for a chance at 1cm. Better to use that money to go to a more skilled implant surgeon.
Just an opinion.

shrunken,
I had revision at 27 months post Titan implant because of crossover of right RTE into left crus which gave me pain when inflated and great deal of discomfort when deflated.
For revision, I had chosen CX. On revision, I went from 18+1 to 21+1.5.
My surgeon was convinced that it was real undersizing.
Even with significant increase in cylinder length, I have no pain nor discomfort regardless of being inflated or deflated.

I don't know if there is a certain length or not for intercourse but I can tell you intercourse requires erection.

MK1965;

I see, your doctor was Clavell. He is a great doctor. He does the sliding technique in case of extreme shortening. Is there a way to consult him via video conferencing?

No one seems to have mentioned using the vacuum erectile device to regain lost length from ED.

I highly recommend doing that and please don't assume you'll lose more length with the surgery.
I even gained a little and with the CX, it wasn't expected.
