Hi brothers,
I believe only here I can get the information I need.
Before having my RP in 2017, I did sperm banking to be able to conceive after surgery. My wife is less than 40y old. We did 2 unsuccessful IUIs, one before and one after the RP. The latter with frozen sperm.
A couple of months ago, my wife's PCP mentioned to her that the chances with frozen sperm are low, which upset both of us.
I recently read about Testicular sperm extraction (TSE). Will this option work with my implant? Or should I not go for it so that I will not be at risk of infection?
I also read on a JHH link that it is (ALMOST) impossible to to ejaculate with the prostate removed. Would this (almost) mean that there is at least 1% chance the sperm can still come out with ejaculation during sex?
Is there anyone on this forum who went through a similar process?
Thank you

I don't have your experience--but here is what I know.
My Urologist--Dr Herati at the Johns Hopkins Brady Urologic Institute has told me previously that he does sperm extraction. He also did my implant surgery. I would not see why a TSE would not work with an implant--but I am sure he could tell you for sure. You will never know until you ask an expert.

Thanks Bob!
Dr. Herati is the first urologist who spoke to me about the implant in Jan. 2020. As I wasn't even considering this option, I said that I may think about it after 5 or 10 years. His reply was, you can enjoy sex now. So, a couple of months later, I thought about what he said, searched the internet, and found FT.
For the implant, the risk of infection when having surgeries close to the implant area is relatively high. So, if it is risky, I will avoid it.

I don't know if you saw this in the bio:
You may not need a surgery to get the sperm.
he has the ability to perform fertility treatments on patients who have experienced spinal cord injuries using nonsurgical methods of sperm extraction.

Not sure but I think that is in reference to vibratory stimulation and electro-stimulation which bypasses the need for communication between the lower spine and the brain for ejaculation. With my DE (somewhat akin to anorgasmia but not quite), I've tried many things to make ejac easier and a less time intensive process, even if non-penetratively. I have purchased the Viberect "ejaculatory-version" device (difficult to use properly due to the thickness of the implant cylinder tips) and I'm considering electro-stim devices (similar to TENS and NMES devices but frequency and amplitudes are adjustable for the purpose of triggering ejaculation).
These methods require a prostate though. As long as the RP did not remove the prostate and only removed the inner surface and there is some degree of ejaculation still present, they might work. The real question is whether the plumbing to the prostate is still present (seminal vesicles and vas deferens, and whether they still function and connect into the urethra via the remainder of the prostate).
