Intro & pulling down the pump.

Postby Cdogg737 » Thu Mar 04, 2021 6:29 pm

Hi guys,

First off just let me say that this forum has been a godsend and the amount of information and advice available is so helpful for anyone who is on the ED journey.

A bit about me, I am 41yrs old, have suffered from ED my entire life, most likely some psychological issue as a physical cause was never found. I could get nocturnal erections and during masturbation (with alot of pressure) but rarely worked during sex.
I started off in my early 20s on the sildenafil/apomorhine lozenges which worked great, but then no longer available, then moved onto Viagra/Levitra/Cialis - all of which had varying degrees of success in my mid to late 20s but then stopped working.
I was then put onto the Caverject injections which were the only thing that ever really worked 100% of the time, but they were a pain to use - the erections were painful and felt unnatural, meaning that orgasm during sex was almost impossible, then finally the availablility of the injections became the problem as they were no longer available in the country (consequence of Brexit apparently).
So I was left with no other options and decided to go for an implant. I am now 10 days post surgery - still alot of brusing, penis is sore, area around the pump is sore, but overall the pain is manageble and I definitely feel like its getting better each day.

I just want some opinions/advice on the pump. Ive been instructed to pull down on the pump as soon as I am able to in order to get it to sit in the ideal position. I began trying to do this yesterday and right now the base of the pump is not sitting against the floor of the scrotum, but its probably only about 1cm above, if that.
My questions are:-
How low is it supposed to be?
Is it a big deal if it doesnt seem like its going to budge from its current position?

My Dr has simply said that he wants me to keep gently pulling down the pump as it apparently can raise up high during the healing process. He didnt really offer any further detail on it. Ive watched the Youtube video from Dr Hakky and it seems like he is applying a fair bit of pressure to pull it down. Im scared if I pull too hard either it will break, or I will - or that maybe I dont even need to pull it any further because its fine where it is right now.

Cdogg737,
Your doctor is spot on. Here's the hard part, pulling down "gently" BUT "firmly". Easy to say. Your body will tell you all you need to know.
So, you're not trying to pull the scrotum down to your knees. Grasp the pump firmly and pull down until you feel a good stretch and hold for a few seconds, then release...repeat, repeat, repeat.
BTW, welcome to the bionic brotherhood. A rock hard erection awaits, in time. BE PATIENT.

PM me any time.

I am 14 days out and facing this very challenge. I have managed to reorient the pump from sitting horizontally to vertically, but it is still sitting high in the back above my ball. I'll keep working on it because where it is right now will be difficult to use.
20 years of severe Peyronie's plaque, 90 curve, hinging and ED. Cost me 1.5" L and 1" G.
Implanted 2/18/21, AMS CX, 18 CM + 3 RTE, penoscrotal. Have gained 3/4". Gay, married, age 68.

Yeah I have a hunch just based on what I have read here that no matter where it sits, it all kinda works itself out in the end, so I am trying to not fixate on it too much, but at the same time I dont want to get to my follow up appointment in a few weeks only to find out that Ive screwed up and made things worse for myself.

Its funny...despite reading the advice on here countless times about just focus on healing for the first few days and not how you think its going to end up, Ive still gone through the whole range of emotions and freak out moments:
*Oh gosh Ive lost too much length, what if it doesnt come back?
*I dont think the tips are up far enough....and why is this cylinder buckling??
*Why can I only feel one cylinder...OMG, was I too small for a titan and he realised that he can only put one cylinder in instead of two??
*Why did he put the pump there, I dont think I will be able to use it, dammit, what have I done??

As each day goes by and I continue to read others posts on here I am becoming more and more convinced that everything is fine and I just need to relax!

LOL...I have been through every one of the emotions and doubts you mention...and more. I'll keep working on it, and hope all goes well when I meet with the AMS rep in 2 weeks for training and hopefully activation to begin cycling. Like you, I'm trying to stay positive and focused on the goal. A dick that does what it is supposed to do.

Cdogg737,

Similar stuff happening to me. Day 6 and very little pain anywhere. But, I've a small sack. One nut or the other is always in Testicular Retraction mode. Makes room for the pump. Following directions to "move the pump around" my pump bulb bottom is pointing up towards my shaft. tubes are pointing down. I'm trying to rotate the pump down toward feet or out front towards the street. Hope that makes sense. I don't try to rotate it to the rear due to tubes. maybe it's normal in a "Mini-Sacker"? LoL Got a call in to the Dr. Office. Maybe this is just strange enough to get an early appointment.

I had Buyers Remorse about 3 hours in. Sun came up in the Morning and Mental and Physical get a Little Better each day. Just as promised by our Bionic Brothers. The best part so far, I don't have to hunt for my dick to take a piss. Woot!

Thanks to everyone posting here.

Hi Cdogg737.
Congratulations for joining the Bionic brotherhood!
I had the same instructions of pulling down the pump, and I am about 6 weeks post implant. I believe 1cm above scrotum floor is an excellent position for the pump. You may need to just pull it down to keep it in that place. Mine is a little higher, but still approachable without a problem. Just keep pulling down gently so that it doesn't move up, and you will be good.

Am I correct in stating that pulling down the pump only applies to the infrapubic procedure? I was under the impression that with the peno-scrotal procedure there is no need to do this because the pump can be installed in an optimal position.
Born 1956, TRT, Peyronies (no improvement after 1 cycle of Xiaflex so discontinued), using alprostadil but have possible VL. Considering an implant. Aquablation for BPH Nov 2 2020, TURP 6/14/2021.

My procedure was penoscrotal and that was what I was instructed to do. The way it was explained to me is that the irrespective of where the pump is first implanted, during the healing process the tissues contract causing it to be drawn upwards.

I have read some other stuff where it says that the skin may attach itself to the pump and so rather than just pulling down, its also good to massage the skin to try to get it free. I tried this last night and it seems to have helped - it almost felt as though the pump was stuck to the front wall of my scrotum, but after a bit of gentle massaging, moving about, 'peeling' etc, it felt like the pump felt a bit more free.

I think as the swelling continues to go down and the pain eases I will just keep doing that each day until my follow up appointment.
To be honest the bigger issue beginning to creep in at the moment is the temptation to start pumping....it feels too hard to press and the Dr did say to wait....but damn Im wanting to try it out!
