I was a grower. 2.5" to 3" flaccid and 6" erect. After implant, I am 6" erect. There (as you suggested) there is NO reason your erect implanted penis should be any smaller than your (immediately pre-op) erect penis, no matter what your flaccid state is. If your surgeon does not understand this, he needs to go back to school or quit surgery. The implant goes into the tunica albuginea up to the furthest extent of the distal tip and down into the furthest extent of the proximal crus JUST AS YOUR CORPUS CAVERNOSUM DOES when it is working correctly.
Study some of the videos on Youtube to understand how the length measurement is taken INSIDE the penis. Then talk to your surgeon about his measuring technique. If he does not explain satisfactorily what (you reported in your post) he said, find another surgeon. You are part of your medical care team and should be included with full participation by all the professional members.
edited to add: Does your case of Hard Flaccid Syndrome affect your surgeon's opinion on your ultimate size?
I don't think the surgeon injects saline into your penis to induce an erection before the surgery. That may be a technique I have not seen mentioned? Injecting an erectile drug before surgery is not uncommon (usually, I believe, in a pre-op appointment, days or weeks before surgery). I never imagined a surgeon would start surgery on an erect penis.
