24 and considering an implant. Hate injections! Here's my story

Hi everyone. I'm turning 24 next month. I'm a gay guy, live in São Paulo, Brazil and have had ED my whole life (venous leak). Been with my boyfriend since I came out, so more than 5 years now. Also I have an open relationship.

I'm writing this to share my story with you so I can have your opinion on my case, and maybe help some young guys who are suffering with ED. Who knows?

But first, let me get straight to the point: I’m strongly considering an implant, after being unsuccessful with pills and after doing injections since July 2016 (bimix and trimix). But I’ll thoroughly comment on everything. The thing is: bimix works for me most of the time (50 units, sometimes more), and trimix makes my penis hurt and I have erections that take too long to go away with it. I would like to just simply have the option to just be able to have sex in the middle of the night without having to get off bed, killing the mood just to have to inject the medication. I could never do that, that’s kind of a dream of mine. And my question is: if bimix works most of the time for me, should I persist with it, even though I hate it and find it very inconvenient (I feel my penis hurts a little when I touch the right side of it from so much injecting I did already)? And should I try the shockwave therapy option before attempting an implant, even though the results are temporary (I talk about it in the text below, it's in bold if you'd like to read about it)?

But anyways, here’s my story to those who would like to read about it. Like I said, it may help some young guys out there.

I have always thought my erections were not good enough (6/10), and I have a congenital curve as well. I can get a natural erection (a crappy one) but can’t keep it, even when masturbating, but couldn’t really tell it was a bad one until I started having sex. Yeah, no matter how much I try. No matter how much therapy I did. Also, sildenafil, tadalafil and lodenafil didn’t work. Sildenafil and tadalafil combined couldn’t keep my erection either. Also had to deal with shitty doctors who just made me feel worse furiously telling me things like “you’re young, that’s all psychological”, barely listening to the things I had to say. Bullshit, I was already reading FrankTalk forums back in 2015/2016 and knew there was actually something wrong with me. And most doctors didn’t even ask me to do a doppler test or blood exams.

Finally in 2016 I decided to see a very good doctor (Dr. Paulo Egydio, you may have heard of him, he is very famous for the penile straightening surgical technique he’s created, Egydio Technique, the one that elongates the shorter size of the penis, instead of shortening the long one to fix peyronie’s/congenital curve). Did the doppler test and found it was venous leak. Diastolic velocity was:

Left > 8.68 cm/s

Right > 8.54 cm/s

Also, he told me I should have a frenectomy (I have an excessive frenum, and a very little phimosis) and fix my congenital curve (45º approximately, to the left, makes my penis fold 90° because I don’t have strong erections). So I started doing injections (bimix first, Papaverine 30 mg/mL, Phentolamine, 1 mg/ml) who sometimes worked well (that’s when I first managed to penetrate in my life, couldn’t do it before injections AT ALL) at about 40-50 units, and sometimes didn’t. Tried a combination of Bimix + sildenafil or tadalafil sometimes, but nothing super different happened. Found out I also had problems with my glans who couldn’t properly swell, and that + excessive frenum + congenital curve + wearing a condom would just make the glans get kinda stuck inside during in and out motions, making me feel no pleasure at all. Yay. Had no surgeries (they were VERY expensive for my family and I didn’t make much money by myself then) and just kept using the injections.

3 years ago, a different doctor then told me to switch to Trimix (Papaverine 12 mg/mL, Phentolamine 3 mg/mL and Prostaglandin 20 mcg/m). Tried 20 units the first time and almost thought I was gonna die because of the pain and extremely hard dick. Then I just kept it at 10 units but found it super impractical to keep it in the refrigerator. It would just make it hard for me to carry it around. Also, had and still have trouble with self-injecting. Sometimes I give giant bruises to my penis. Sometimes I just inject it and it just hurts my penis because apparently I don’t pick the right spot for the injection. I switched back to Bimix for the inconvenience of Trimix, but still find it hard to self-inject (and still have all of the previously mentioned problems). So then, last year I started considering an implant because first I hate these injections and it just KILLS my mood to ask the person I’m about to have sex with to give me a couple minutes to go to the bathroom “to go clean my contact lens” or whatever idea I come up with (I don’t tell them I’ll be self-injecting Bimix to get an erection, of course), which makes them just wait for me. Anyways, I don’t like injections at all, and even though I’ve been dating my boyfriend for more than 5 years, I feel this waiting kills the mood for him too (can’t complain about him though, he is SUPER kind to me, it’s just that I really hate these injections).

Anyways, found this other VERY good doctor, he’s a urologist and an andrologist. He was super kind to me (as opposed to many others who very often just made me cry for being treated so bad) when I went to see him. I stayed in his office talking to him about the implant and treatment options for more than an hour (here’s his website for my fellow Brazilian friends https://www.drmarcotuliourologista.com. ... arco-tulio). He recommended I try shockwave therapy, saying it would may improve my erection 50-60%, but the results won't last forever. He says it lasts for maybe a year for some, maybe less for others, maybe more for others. The thing is: I’m still gonna have to keep injecting myself with bimix, and also there’s the psychological damage ED has done to me, and I don’t think I would ever be able to make pills work with me (even if -just if- my erection really does get better with this shockwave therapy).

Long story short: I’m reading about pros and cons of an implant. I’ll be seeing this doctor again next month and have prepared a huge list of questions. He says he can perform Egydyo’s Technique on me, so my penis won’t be curve anymore, it will become longer actually post surgery, and he can also remove the foreskin excess I have, therefore making the glans be freely exposed. Also, he can perform the implant surgery on me. He recommends – by the way – Titan OTR zero degree (Coloplast). It’s an inflatable one. He says an implant won’t solve the glans swelling problem, but would make me never be able to worry about an erection again, and all the other problems would be fixed as well. He just said I should try the shockwave therapy first, but would perform the implant surgery on me if I decide to.

So yeah, that’s my story and these are my problems. I would love to hear your opinion on it and would gladly talk to anyone out there about the issues I am facing, mainly the young boys who are going through the same thing I am facing.

I am much older but I went through all the pills,injections,etc. I got an implant in 2013. I’ve had several repairs , my luck is always bad, but I’ve never looked back. I strongly recommend an implant to anyone that’s tried other avenues. I love my implant and would never go back to injections over the implant. Just my two cents worth

I feel maybe implants do become kind of overkill when injections works.
There are some really cons with implants; price, needed revisions, loss of size, not getting effect from new ED treatments in the future,
while Injections are mostly inconvenience, source to possible fibrosis and psychological stress (last one is especially something which botters me).

It's hard to choose.

May I suggest you keep trying trimix but inject it slowly, I found that worked for me to stop the pain. As for bruising you just need to miss the small veins. I miss my natural dick but nothing worked so I went for the implant. Works great.

Yeah... it is. I just keep thinking about this scenario that I’ll away be ready for it, not having to interrupt anything to just self-inject and will never ever have to suffer with ED again. My boyfriend also told me “well, if you hate injections that much, you should get an implant soon. You’re still young, you might miss the best time of your life” and I guess he’s right.

You said you miss your natural dick... what do you mean? What are the differences?

Yeah, same. I mean, if I adjust the dosage, it doesn’t hurt. But I would get these erections that would take too long to go away.

What happened? Did it stop working? Did your body reject it?
My biggest fear is rejection. Do you think since I’m 24 and have no other health issues I would have a smaller probability of having a rejection?
