For penile prosthesis the terms are used when measuring for the length of the cylinders. When the surgeon measures, they must pick a physical location on the penis as a reference point. This is usually part of the corporotomy. In my diagram I'm using the left of the corporotomy as the reference point. The surgeon will insert a measuring rod inside the corpora to the head of the penis (Distal) and record the length. Then turn the measuring rod around and insert till the bottom of the crus is determined. (Proximal) Then these two measurements are added to determine the length of cylinders required. The surgeon repeats this procedure for the other corpora.

The surgeon could choose the right side of the corporotomy or a mole on your penis as a reference point.

So the distal and proximal measurements have no correlation to penis size.

My analogy was to make it easier to understand but to be literal, DISTAL has the same root as DISTANCE and PROXIMAL has the same root as PROXIMITY. DISTAL is away from the center and PROXIMAL is toward the center.

By the way, the reference point on your drawing was a little bit incorrect. That would put the corporotomies within the penetrating portion of one's penis but in reality they're just proximal of the fat pad or right at the fat pad in my case. Hopefully nobody would ever want the corporotomies along the exposed shaft at the base of their dick.
