Hello I am in contact with 2 surgeons for my implant.

Both of them are real good surgeons. Only thing which is different is that one of them is only doing it infrapubic and the other penoscrotal.

I guess the penoscrotal version is more painful after surgery but when healed everything will be better.
And I guess the infrapubic installation is only for the surgeon to get it in there FASTER and EASIER.

Would be great if a infrapubic an an penoscrotal patient can just tell me some infos!

I am also interested if maybe the infrapubic installation reduce the sensibility of my penis?

WOULD BE GREAT FROM YOU TO JUST QUICK GIVE ME AN ANSWER TO MY QUESTION!

Hi rocco84

I had penoscrotal implant surgery on 30 Jun 2020. Personally, I can't recommend it. My testicles were incredibly swollen and black for almost 4 weeks. I couldn't start cycling until 5 weeks, which could decrease penis size. (Based on what I've read, waiting 5-6 weeks to start cycling is associated with reduced size outcomes.)

If I had to do it again, I would very seriously consider infrapubic instead.

Maybe someone else has more info, especially about infrapubic.

Hey rocc84!

I had my AMS LGX implanted infrapubicly. I'm happy to answer any specific questions you may have about the infrapubic method.

An advantage of the infrapubic method is the reduced swelling in the scrotum during recovery. This usually allows the patient to start cycling sooner than if the patient underwent the penoscrotal approach. I was able to start cycling my implant 5 days after surgery. A lot of surgeons believe that early and aggressive cycling is the key to avoiding a loss of length with the implant. Thankfully, while I was initially about half an inch shorter when inflated, frequent cycling eventually stretched me back out to my lifelong erect length. I also gained some girth thru cycling, so yay!

I can confirm that the infrapubic installation does not reduce sensitivity of the penis, or at least it did not in my case. I asked my surgeon, Dr. Karpman about this as the idea of loosing sensitivity really terrified me. Dr. Karpman said that experienced infrapubic surgeons know exactly how to avoid damaging any nerves that would cause sensitivity loss, and that as long as your surgeon is capable and experienced, sensitivity loss is not a concern with the infrapubic approach.
You say that the infrapubic surgeon you talked to is a good surgeon, so I don't think you have much to worry about in regards to sensitivity loss, I'm sure he knows how to stay clear of any potential nerve damage.

The only area of my implant that I am somewhat unhappy wish is my pump position. It's too far forward and not hidden as well as I believed it would be, and sometimes the feeling of it in my scrotom bothers me. I know that after surgery it was placed correctly, I felt it in my sack the morning after, but I believe during the healing process it seems to have shifted into it's new position. I'm not sure if this is a result of Dr. Karpman messing something up with the infrapubic approach, or whether I did something wrong during the healing process that caused it to shift. Either way, it's not a big deal for me and overall I am very happy with my implant.

On rereading your post, I'm curious as to where you got the following information. You wrote:

"And I guess the infrapubic installation is only for the surgeon to get it in there FASTER and EASIER." (as in they only do infrapubic because it's easier and faster for the urologist?)

It almost sounds like someone who does the penoscrotal technique is badmouthing urologists who use the infrapubic technique.

I really am interested in hearing where you learned that; if you can respond with that info, I'd appreciate it.

My implant was done infapubic in January 2020. No pain hardly noticeable. Hard to believe but it’s true. I healed for six weeks then cleared for sex. No sex for six weeks was the hardest part for me. Everything works great no complications. I still see minimal scaring which is hardly noticeable because of pubic hair.
66 yr old male married 36 yrs use trimix four yrs, cilais and Viagra. trimix work well developed scarring on both sides had implant 1/9/2020 at UT Med Ctr, Knoxville, TN Dr. John Lacy.

rooco84, I forgot to say that my implant was done infrapubic. That’s the way I wanted it done. My surgeon in the past did them penoscrotal, but he feels that the surgery is easier on the patient using this method. Just use a lot of ice for a few days to lower the swelling. I didn’t have a lot of pain. Tylenol was all I needed at times for the 1st week. Take care.

I guess someone has to weigh in on the peno-scrotal method.
I had it and had minimal swelling and no bruising and not very bad pain.

I like that the tubing just comes straight down from the cylinders and is noticeable, but not problematic.
I think a bit more skill may be needed to install the reservoir.

Inject testosterone weekly.
Implant on 1/22/19 by Dr Avila.
Scrotal, hor. incision just over 1"
18cm AMS 700 CX, 3.5cm RTE 100cc res
Gleason 6 prostate cancer. Monitoring it for now.
Update: On my last biopsies the cancer wasn't found.

I had the penoscrotal. No issues at all. No pain past day 2. No swelling. No bruising. I actually would prefer it from what I read about both.
18 cm plus 1 rte titan installed March 2019. Revision March 2020 by Dr. Andrew Todd, Richmond KY. He replaced the titan with an AMS 700 LGX 18 cm cylinder plus 2 rte for 20 cm total length.

I have had both. The first in 2013 was penoscrotal. I did lose a little length but it went well. I had the infrapubic in 2017. No problems much with it either. I liked the penoscrotal approach better and will undergo that again this Thursday. The infrapubic leaves a really bad scar which is not an issue unless you like to keep shaved below. If you have pubic hair it is not visible but I was shaved until that surgery and then had to let the pubic hair grow to cover scar.
Revision 2/13/24 Dr Tariq Hakky, Atlanta (Great Doctor) Titan 24XL no Rte’s. I’m 62 married. I have had numerous revisions due to product and body failures. Always had a Titan ipp.
