What was your girth before the implant?

I think that it is hard to say for sure. But more saline might not be your problem. Small chance that your reservoir is too small. You'll be hard pressed to get a bigger one cause they hardly ever fail. All of this is based upon you seem to be saying that it only took 3 months to grow enough to run out of saline.
Is there a chance that you have a leak? I'm betting that if you slowly get less girth & length after pumping that you have a leak rather than an undersized reservoir. Any chance that tubing got nicked on your later surgery?
Your surgery notes should give you some clues about the first guess. No doctor will admit to the second possibilty.

I shouldn't comment on the Titan cause most of my research has been centered around AMS. I'm sure that AMS has at least 2 sizes. Hard for me to see the logic of implanting a 26cm implant & not completely filling the reservoir.
Have you talked to your doctor about it? Have you read your surgery notes?

Just checked. AMS has 2 options, a 65ml & 100ml .

My Titan has a 125cc

It was 6.5” a few months pre op. About a 25% loss of size including length lost post op. It’s been 1 year post op and the implant seems just as hard/long/girth as it did 3 months post op. The only thing that’s noticeably changed is how easy it is to pump up compared to when I first started cycling it. I didn’t know the resivor came in different sizes. Would be nice to switch mine out with a larger size and maybe gain some girth back. I think I asked the surgeon a while back and he said that being able to use 100% of the saline was not a issue. My guess at the time was it was common practice to under fill it to a certain amount based on implant size (26 cm) but I really didn’t know for sure.

You have a 26 cm implant so he would have used the largest reservoir and filled it. Otherwise you could never fully inflate. I have an 18 CM LGX with a 100 cc reservoir filled to 80 cc. I never use all of the fluid and I make 6.5 most times but not always.

Like Doug, I have 18 cm Titan and reservoir filled with 80 ml of saline.
I never run out of fluid when I was able to inflate it to ax 100%.
For the last 3 months, I never was able to inflate it more then 50-60% because of severe pain and malposition of my right cylinder.
