VED: implant inflated or not?

I wonder what’s the correct procedure. When using a VED, do you guys inflate the implant? What do the doctors recommend? In my opinion a fully inflated implant won’t make a lot of sense, it would interfere with the flow of blood.

If I would inflate more than 50% the dick won’t be able entering the VED anyway.

According to Dr. Perito, inflate the implant and then attach VED and inflate VED until one pump before uncomfortable.

Well, IMHO, inflating implant (or not inflating) for use with a VED is dealer's choice.
Either way, you will get the blood flowing. why wouldn't you?
I have had a semi and now have a 3-piece.
For the semi, I was using a VED 3 weeks prior to surgery to getting 3 piece. IMHO, it helps with those mysterious pains. For the semi, you don't get to pump, everything is right there! Just lift up, insert.
For the 3 piece, I just had surgery in March (and Dr's Telehealth appt the AM today), things are still too painful for insertion into sleeve (and I am uncut). The pumping stretches the inside of my dick, and pumping is painful when I really stretch things. Inserting dick into sleeve of VED was not going to work, so I took out sleeve and have inserted and used the VED successfully. FYI, Dr. told me that the force needed for penetration on sleeve vs. (actual) vagina is 12 lbs vs. 3 lbs. I mentioned oral was painful, and all he said was I was still healing.

I told her that I watched the Patient Training Resource Video put out by AMS and in that video it recommends that the VED be used. She was shocked. Further, she says that the only reason they don't recommend it is because it has not been tested. The 4 top implant surgeons in North Americas recommend it as long as there is not a ridiculous amount of pressure. The closest I got to a straight answer is:
"Fully pump up you IPP, then attach the VED. Pump the VED until it's comfortable BUT one more pump would NOT be comfortable. Leave it like that for 30 minutes". :)

Quite interesting. However if I fully pump up my IPP there is no way that it would enter the cylinder. Way too thick my dick. I tried it without any pumping up and it worked very well.

For a long time I was under the impressing that with an IPP one cannot use a VED, it might even be dangerous. Seems I was wrong.

What this means is that the wrong size of VED cylinder is being used. Get a larger diameter cylinder (LeLuv makes all sorts of diameters and sells on Amazon). If the issue is the inside diameter of the silicone sleeve at the end, take it off or get one with a bigger inside diameter hole.

I have a question about the Leluv pump: Can it be used under water?

I have had MUCH better results using a VED under water than I ever did in an air environment.
Born 1947. Live in Utah. Married 1995. 5'8". 160 lbs.
Health is very good. ED began from venous leakage (2003).
Used Cialis (2003-2017), penile injections (2018-2019), 4 implants (2020-2021), now have an AMS 700 installed.
Hobbies: Only one, my wife.

niarceel, I posted this to you in PM too but I'm adding it here too for others who may be curious.

LeLuv's cylinders are water-tolerant because they're only polycarbonate (think "acrylic" or "plexiglas") with plastic and steel connector at the top. The only issue would be the pump. Since they are sold separately, you would pair the cylinder with your choice of pumps. I had a brass trigger-pull type pump with vacuum gauge, like the ones used for automotive brake bleeding. I bought the entire rig on Amazon by the piece. I guess if a vacuum gauge isn't needed, the squeeze bulb might work but since it's moving water instead of air, I don't know if it would be able to handle the additional viscosity of the medium (water is thicker than air, or more viscose). If you can find a hand-trigger water pump, preferrably with a gauge, that would be ideal.

Thanks, Waynetho.

After reading your post, I got on the Amazon website, reviewed the output from the "Leluv" search too quickly. I thought the pump was the Leluv unit you spoke of instead of the cylinder.

In the recent past, I have used a plexiglass, $20 "wonder pump" with a rubber bulb attached to it, no gauge. Having a gauge was not important to me after reading a physician's recommendation for using VED's: "Vacuum until a slight stinging sensation is felt, nothing more. Then, hold for 10 minutes per day."

That advice worked really well. I used a VED pre-IPP surgery to ensure a best result for cylinder implant length. The greatest improvements in penile dimensions were achieved at ~3 week mark. The CV19 panic postponed my surgery, so I ended up using a VED for about 3 months. The results were beyond anything I expected. I went from 5.5 inches in length to just short of 6.5 inches in length. I wished I'd measured the diameter of my penile shaft; I didn't. But I can tell you that it made a big difference which my wife noticed during both oral sex and intercourse. If I'd known results like this were possible, I would have used a VED decades ago.

The big breakthrough was going from an air environment to a water environment. All the issues I had with seal leakage and bulb inefficiencies vanished in water. The vacuum effect was so strong in water that I had to be very careful not to over-create vacuum pressure for fear of injuring my penis.

IMO it works better if not inflated. When inflated many blood vessels get compressed. Much more blood enters the penis if deflated. That’s how I do it now.
