Revision by the best Dr.Karpman

Postby medhatg » Wed May 13, 2020 8:57 pm

I just had my revision done by the one of the renowned Dr. Karpman and I'm quite pleased. To provide specifics:
- Dr. Karpman and his office staff were the best ever during the whole process. The hospital was amazing too and the concept of staying overnight made a huge difference especially that I was alone.
- Dr. Karpman replaced my failed LGX 21+4 with CX 24+2. Dr. Karpman has the same opinion as Dr. Eid that the less the RTE's, the more rigid and stronger the implant is.
- Boston Scientific rep worked with the hospital and Dr. Karpman's office on the warranty specifics.
- Dr. Karpman used infra pubic and draining line over-night. I could barely see any bruising compared to my first surgery where the surgeon using scrotum incision.
- I was fully inflated with my penis wrapped for 24 hours after the surgery, then at 70% for another 2 days.

Happy to answer all questions and will try to post updates.

Congratulations on your revision!

Something I've been wondering that maybe you can answer... When someone has an implant installed through penoscrotal incision and then gets a revision via infrapubic incision, how do they get the previous implant out? Do they have to do a penoscrotal cut to remove the old one or does it all come out through the top incision? It sounds like it would be next to impossible to remove the implant from above when the original corporotomies were underneath (and the tubing going through the original corporotomies).

Dr. Karpman removed my broken implant through the infrapubic incision without touching the scrotum. Not sure how he did it, but I could ask in my post-op next week.

I guess theoretically, as long as the stitches and original corporotomies are still in good shape, the doctor could make new corporotomies on top of the corpora, and ease the old implant cylinders out the top, pulling the old tubing through the small hole they used in the old incisions. Then it would be a simple matter of cutting the tubes to get them out completely, pulling the same tubings back the other way through through the same holes when removing the pump. Then perhaps one or two stitches in the holes left by the old tubing (done from the inside the corpora by way of the new corporotomy) to seal them up.

Then "bingo, bango, bongo", he's ready to install the new implant.

Two questions:

1. How long did your previous LGX last and what broke that you required a revision?
2. Why did Doc decide to give you CX? Why not replace it with another LGX?
A young guy in his early 30s from South Asia. Implanted Titan 22cm by Dr. Tariq Hakky in Atlanta (Dec 2020). Amazing doctor! No pain during recovery. My size is 5.75” x 4.6” which is better than pre-OP size of 5.6” x 4.5”. Hoping to gain more in future.

The LGX only lasted 2.5 years. The consensus that I got from talking to Dr. Karpman as well as Dr. Perito and Dr. Christine was that the LGX was not a good fit for my anatomy especially with 4 rtes. Instead of 21 LGX + 4 RTE, Dr. Karpman used 24 cm CX + 2 RTE. He said that the CX has better rigidity and strength than the LGX.

Awesome, that makes a lot of sense. I've one more question - does the CX model retract when you deflate as LGX did? I heard the LGX model's benefit is that it can retract and expand as you deflate and inflate, thus mimicking the natural penis.

It's not that the LGX retracts when you deflate. It's that it expands, up to 20% when you inflate. A 20cm implant may increase in length up to 24cm when fully inflated (dependent on the individual's specific anatomy).

That's correct. However, when I had the LGX, when deflated, it looked pretty normal. However, I'd rather have a more durable, rigid implant with longer life span.

Great answers, learning a lot! Waynetho, can you tell me if your penis during deflate state gets in between your legs when wearing jeans. I agree with medhatg about the durability of CX with a longer life span and that's why I am inclining more towards it. There are just two things that worry me if I go with CX:

1. My 5 & half inches penis size being stuck between my legs or touching end of jeans because it won't retract, not even a little bit, making it difficult to wear jeans.
2. The kink in the shaft that may hurt my penis long term

Any ideas about these two things?
