A common injury or effect from overuse or excess vacuum of a VED is called "donut dick". The glans swells up and looks like a doughnut on the end of the penis. I've not experienced this per se but I got dangerously close once. The reports I've read state that most of the time, "donut dick" will revert to normal after a few days of recovery while not using the VED. This is why you need to make sure the VED you use has a vacuum gauge and you don't use too much or for too long at any one session.

Well, there are guys out there that REALLY use a VED way too much and their dicks look funky.

So what is it exactly that atrophies after you get ED? Is it the cavernosa, the tunica, the suspense ligament? I know it happens, but I've never understood what exactly shrinks.
60 yrs old. Gradually worsening ED for 10 years. Pills and trimix not working well anymore. Will need an implant in the near future.

That is a very good question. One I have not heard (or considered myself) before. Which is odd, because I am a curious guy.

These are my speculations: Do not take them as truth. I am not any sort of medical professional.

I suspect, in the most extreme cases, the cavernosum tissue may atrophy or die off from not getting oxygenated for a long period of time. Nocturnal erections and normal blood flow usually take care of that. This a a WILD speculation.

The Tunica is a fibrous tissue that, when fully inflated by the corpus cavernosum hits a limit of expansion. That hard, firm limit is why an erection is hard and rigid. But that tissue also shrinks down (a degree of elasticity at any size under its limit). If not stretched out to its limit periodically (usually by nocturnal erections and sexual activity) the elasticity goes away. Call it atrophy, but I am sure there is another medical term more appropriate.

I don't thknk the suspensory ligament is involved at all.

Like I said, speculations. I look forward to more authoritative answers, especially if they are sourced.

I believe the main thing that happens with the suspensory ligament is loosening over time but this isn't necessarily an ED issue but rather age related stretching. What occurs when the ligament loosens is the erection angle will start to drop from where it was in a man's 20's and in my case, after implantation I have an erection angle of about 4 o'clock or 4:30 which equates to between 60 and 45 degrees from straight down or 30-45 degrees from horizontal (think of a clock, where is the 4 and halfway between 4 & 5?)

In my case, I believe I have a stretched or damaged suspensory ligament along with either an insufficient crus penetration of my implant or a short crus (1 to 1.5" max, just under front pubic bone).

Either way, the effect is that I don't have the angle that points up (1:30-2:00) or straight out (3:00). It points about halfway between out and down when inflated (45 degrees from horizontal, 45 degrees from straight down).

Most seem to gain nothing using the std VED protocols. I used max pressure for short times repeatedly. Took me 60 days to regain 1 5/8 inches. In the end I blistered myself not fun. DO NOT HOLD MAX PRESSURE FOR MORE THAN 1 minute. Good luck[/quote]
When I first started pre-implant VED therapy I used to pump up and hold the vacuum for a long time. I did not have the "blistering" or edema you mention. Not until I took the pressure too high. Then, I did. The edema was not too bad. I got over-enthusiastic about my size inside the vacuum tube. I was larger than I had ever been in there.

It resolved after a few days of not vacuuming. After that, I limited the pressure, not time time, inside the vacuum tube.

I have no idea what would have happened if I had done (what I think) you do, pump up to a high vacuum, but limit the length of time. I did not try it that way.

Eventually, I switched to a protocol that involved pumping up, holding for a short time and releasing the vacuum and repeating that cycle for a half-hour twice a day. But I was careful not to go to as high a vacuum as had given me the edema (in the extended minutes duration) before.

So, I wonder if I would have been able to get any increase in size. As it is, my penis is the same size as it was pre-op (and pre-E.D., thankfully I did not have atrophy going in).

Recovery of lost (erect) size seems affected by how long ago the size was lost. Recently lost size seems a lot easier to recover. If one starts vacuum therapy as soon as E.D. takes away nocturnal erections, it seems to me that size loss can be avoided altogether.

(edited after Mk1965's post) My comment applies only to size loss not due to any surgery, injury, disease process or other cause other than loss by non-existence of the periodic erections that men not suffering from E.D. experience. (i.e. nocturnal erections, etc)

After prostate removal surgery for prostate Ca, guys are left with instant loss. I discovered mine in recovery room. My penis was significantly shorter right after surgery. VED was useless same as Cialis and Viagra.

My implant brought my penis back to its original length (5" hard), and the soft length went from 2" to 3.5". :D

My loss occurred over a lifetime very slowly. By 35 I had lost about an inch. Maybe less. But many years later 2 years of impotency brought me down to a 1 5/8 inch total loss. I regained that all with 2 months of vigorous VED. So decades of shrinking undone in 60 days.
Studies I have read say that the 1 inch loss from shortening the urethrae during a radical prostectomy comes back naturally within one year IF YOU CAN STILL HAVE HARD ERECTIONS. If not I have to assume that a good VED protocol would do the same.
Word of advice if you use VED keep this on hand just in case. A pack of condoms and a small bottle of vitamin E oil. If your blisters do not pop just let then heal. If they do pop wash with clean water put the oil in a condom and tape it on. Change daily.
